集束化护理联合气道分级管理模式在新生儿呼吸困难患儿持续气道正压通气治疗中的应用

摘要 目的探讨集束化护理联合气道分级管理模式在新生儿呼吸困难患儿持续气道正压通气(CPAP)治疗中的应用效果。方法选取2018年1月至2021年1月宜春市人民医院收治的84例新生儿呼吸困难患儿作为研究对象,按入院先后顺序分为对照组和观察组,每组各42例。两组患儿均采用CPAP治疗,对照组患儿采用集束化护理,观察组患儿在对照组基础上联合气道分级管理模式进行护理。比较两组患儿的护理总有效率、呼吸相关指标以及血气指标。结果观察组的护理总有效率为93.86%,高于对照组的73.81%,差异有统计学意义(P<0.05)。两组患儿护理前的呼吸频率(RR)、氧合指数(OI)比较,差异无统计学意义(P>0.05);观察组患儿护理后的RR低于对照组,OI高于对照组,差异有统计学意义(P<0.05)。两组患儿护理前的动脉血氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))比较,差异无统计学意义(P>0.05);观察组患儿护理后的PaO_(2)高于对照组,PaCO_(2)低于对照组,差异有统计学意义(P<0.05)。结论集束化护理联合气道分级管理模式在新生儿呼吸困难患儿CPAP治疗中的效果好于单一采用集束化护理,护理总有效率能显著提升,患儿呼吸频率、血气水平等指标明显得到改善。 Objective To explore the application effect of cluster nursing combined with graded airway management mode in the treatment of continuous positive airway pressure(CPAP)in neonatal children with dyspnea.Methods A total of 84 neonates with dyspnea admitted to Yichun People′s Hospital from January 2018 to January 2021 were selected as the research objects and divided into the control group and the observation group according to the order of admission,with 42 cases in each group.The children in the two groups were treated with CPAP,the children in control group received cluster nursing,while the observation group received airway hierarchical management mode on the basis of the control group.The total nursing effective rate,respiratory related indexes and blood gas indexes were compared between the two groups.Results The total effective rate of the observation group was 93.86%,which was higher than that of the control group(73.81%),and the difference was statistically significant(P<0.05).There were no significant differences in respiratory rate(RR)and oxygenation index(OI)between the two groups before nursing(P>0.05).The RR of the observation group after nursing was lower than that of the control group,and the OI was higher than that of the control group,and the differences were statistically significant(P<0.05).There was no significant difference in arterial partial pressure of oxygen(PaO_(2))and partial pressure of carbon dioxide(PaCO_(2))between the two groups before nursing(P>0.05).The PaO_(2) of the observation group after nursing was higher than that of the control group,and the PaCO_(2) was lower than that of the control group,and the differences were statistically significant(P<0.05).Conclusion Cluster nursing combined with airway hierarchical management mode can achieve better effect than cluster nursing alone in CPAP of neonates with dyspnea,which can significantly improve the effective rate of nursing,RR and blood gas indexes.
