摘要
目的探讨大黄牡丹汤治疗晚期转移性结直肠癌的疗效及对血清肿瘤标志物水平的影响。方法选取扬州市中医院2018年1月~2020年12月收治的晚期转移性结直肠癌患者60例作为研究对象,根据随机数表法把患者分为试验组与对照组,各30例,对照组给予常规化疗,试验组在对照组的基础上给予大黄牡丹汤治疗。观察试验组与对照组的疗效,治疗期间两组患者出现的不良反应情况,治疗前后检测血清癌胚抗原(CEA)与糖链抗原(CA)242含量,治疗前后两组患者卡氏评分(KPS),治疗后随访1年,记录患者的中位生存时间与中位无进展生存期。结果治疗后总有效率比较,试验组高于对照组(P<0.05)。试验组治疗期间的呕吐恶心、贫血、白细胞减少、中性粒细胞减少等不良反应发生率都低于对照组(P<0.05)。两组治疗后的KPS评分高于治疗前,且试验组高于对照组(P<0.05)。治疗后,两组患者血清CEA与CA242水平均低于治疗前(P<0.05),试验组低于对照组(P<0.05)。所有患者随访1年,试验组的中位生存时间与中位无进展生存期都长于对照组(P<0.05)。结论大黄牡丹汤治疗晚期转移性结直肠癌能提高治疗疗效,减少不良反应的发生,抑制血清CEA与CA242的释放,改善患者的生存质量,延长患者的生存时间。
Objective To investigate the curative effect of Dahuang Mudan Decoction in the treatment of advanced metastatic colorectal cancer and its influence on the level of serum tumor markers.Methods A total of 60 patients with advanced metastatic colorectal cancer who were admitted to Yangzhou Hospital of Traditional Chinese Medicine from January 2018 to December 2020 were selected as the subjects.All the cases were divided into the experimental group and the control group with 30 cases each groups accorded to the 1:1 random envelope drawing principle,the control group group were given conventional chemotherapy,and the experimental group were given Dahuang Mudan Decoction on the basis of the conventional chemotherapy group’s treatment.The curative effects of the two groups were observed,the toxic and side effects of the patients were observed during the treatment,the serum carcinoembryonic antigen(CEA)and carbohydrate antigen(CA)242 contents were detected before and after treatment,and the patients were evaluated for Karnofsky score before and after treatment(karnofsky performance status,KPS),followed up for 1 year after treatment,and recorded the survival of patients.Results After treatment,the total effective rate of the experimental group were higher than the control group(P<0.05).During treatment,the incidence of nausea and vomiting,anemia,leukopenia,and neutropenia in the texperiental group were lower than those in the control group(P<0.05).The KPS scores of the experimental group and the control group after treatment were higher than those before treatment,and the experimental group was higher than the control group(P<0.05).Serum CEA and CA242 levels after treatment in the two groups were lower than before treatment(P<0.05),and the experimental group were lower than the control group(P<0.05).All patients were followed up for 1 year,and the median survival time and median progression-free survival of the experimental group were longer than those of the control group(P<0.05).Conclusion Dahuang Mudan Decoction in the treatment of advanced metastatic colorectal cancer can improve the therapeutic effect,reduce the occurrence of toxic and side effects,inhibit the release of serum CEA and CA242,improve the quality of life of patients,and prolong the survival time of patients.
