摘要
目的:探讨超声造影(CEUS)参数联合声触诊组织量化(VTQ)技术在鉴别肝细胞癌(HCC)及肝血管瘤(HH)中的价值。方法:选择医院收治的86例经病理结果证实为肝脏占位性病变患者,根据病理分型将其分为HCC组(45例)和HH组(41例)。两组患者均进行CEUS和VTQ检查,对患者CEUS不同病理参数的始增时间、始增强度、达峰时间和达峰强度,以及VTQ相关参数的病灶剪切波速度(SWV)值、背景SWV值变化进行分析;分析CEUS、VTQ以及联合检测HCC和HH的诊断效能。结果:HCC组患者CEUS的始增时间及达峰时间明显低于HH组,差异有统计学意义(t=5.776,t=8.500;P<0.05)。HCC组患者病灶SWV值及背景SWV值明显高于HH组,差异有统计学意义(t=7.083,t=7.007;P<0.05)。HH组中CEUS联合VTQ检测出2例HCC,其诊断灵敏度和特异度分别为95.65%和97.50%;CEUS诊断分别为86.67%和85.37%;VTQ诊断分别为86.96%和87.50%。结论:CEUS联合VTQ技术鉴别HCC及HH中可提高诊断灵敏度,两种检测方法互补性强,可提高鉴别病变组织的准确率,具有较高的临床应用价值。
Objective: To investigate the value of contrast-enhanced ultrasound(CEUS) parameters combined with virtual touch tissue quantification(VTQ) in differentiating hepatocellular carcinoma(HCC) and hepatic hemangioma(HH). Methods: 86 patients with space occupying lesions of liver confirmed by pathological results were selected and they were divided into HCC group(45 cases) and HH group(41 cases) according to pathological type. CEUS and VTQ were performed on both groups. And the changes of onset time, starting intensity, time to peak and intensity to peak of different pathological parameters of CEUS on patients, as well as focus shear wave velocity(SWV) value and background SWV value of VTQ related parameters were analyzed. And the diagnostic efficacy of CEUS, VTQ and combined detection of them on HCC and HH were further analyzed. Results: The onset time and time to peak of HCC group were significantly lower than those of HH group(t=5.776, t=8.500;P<0.05). The focus SWV value and background SWV value in HCC group were significantly higher than those in HH group(t=7.083, t=7.007, P<0.05),respectively. In HH group, 2 HCC patients were confirmed by CEUS combined with VTQ, and the sensitivity and specificity of that were 95.65% and 97.50%, respectively. The sensitivity and specificity of CEUS diagnosis were 86.67%and 85.37%, respectively, and the two indicators of VTA diagnosis were 86.96% and 87.50%, respectively. Conclusion:CEUS combined with VTQ can improve the diagnostic sensitivity in the identification of HCC and HH, and the two detection methods are highly complementary, which can improve the accuracy of distinguishing the lesion tissue, and it has higher clinical application value.
