摘要
目的 探讨术前呼吸功能锻炼对老年患者行心脏瓣膜置换术肺功能的影响。方法 选取本院接受瓣膜置换术的老年患者98例,随机分为实验组(49例,术前呼吸功能锻炼)与对照组(49例,术前常规护理),比较两组患者术后呼吸功能与肺部相关并发症发生情况。结果 实验组患者术后肺泡-动脉血氧分压差、氧合指数较对照组显著改善,肺不张发生率较对照组低(P<0.05),两组患者术后ICU时间、呼吸机辅助时间和肺部感染发生率无显著差异。结论 术前呼吸功能锻炼对老年患者接受心脏瓣膜置换术术后恢复有益,值得进一步推广应用。
Objective To investigate the effect of preoperative respiratory function exercise on postoperative pulmonary function in elderly patients undergoing heart valve replacement.Methods Ninety-eight elderly patients undergoing valve replacement in our hospital were randomly divided into experimental group(49 cases,preoperative respiratory function exercise) and control group(49 cases,preoperative routine nursing).The postoperative respiratory function and pulmonary complications of the two groups were compared.Results The alveolar arterial oxygen partial pressure difference and oxygenation index in the experimental group were significantly improved compared with those in the control group,and the incidence of atelectasis was lower than that in the control group(P<0.05).There was no significant difference in postoperative ICU time,ventilator assistance time and the incidence of pulmonary infection between the two groups.Conclusion Preoperative respiratory function exercise in patients with valve replacement can improve their pulmonary function,decrease ventilator assist time after surgery,prevent complications and improve curative effects.
