Distribution and Influencing Factors,Disposal and Drug Treatment of Adverse Transfusion

摘要 目的:探讨发生输血不良反应的分布特点及影响因素、处置及药物治疗。方法:采用回顾性分析,将2017年10月—2021年10月在本院进行输血的11236例受血者作为研究对象,对输血不良反应的发生情况进行调查,根据不良反应发生情况,将出现不良反应的受血者设为观察组,将未出现不良反应的受血者设为对照组,分析其临床资料,总结发生输血不良反应的分布特点及影响因素,并给予相应的处置措施和药物治疗。结果:11236例受血者中发生输血不良反应的共72例,占比为0.64%,其中输血相关性溶血2例,发热反应31例,过敏反应39例。输血不良反应多发生于输入红细胞悬浮液的受血者(占比为31.94%)、输入新鲜冰冻血浆的受血者(占比为26.39%)和输入血小板的受血者(占比为36.11%)。经单因素和Logistic多因素分析显示,输血次数、过敏史、输血史以及原发性血液疾病是输血不良反应发生的独立危险因素(P<0.05)。结论:输入红细胞悬浮液、新鲜冰冻血浆以及血小板易引起输血不良反应,且多见于发热反应和过敏反应,通过采取积极有效的处置措施和药物治疗,可明显降低不良反应发生风险及危害。 Objective:To explore the distribution,factors,disposal and drug treatment of adverse transfusion.Methods:Through retrospective analysis,11,236 blood recipients from October 2017 to October 2021 were investigated as the study objects.According to the occurrence of adverse reactions,the blood recipients were set as the observation group and the control group.The clinical data were analyzed,summarized the distribution characteristics and influencing factors of adverse transfusion reactions,and gave corresponding disposal measures and drug treatment.Results:Among the 11236 recipients,there were 72 cases of adverse transfusion reactions,accounting for 0.64%,including 2 cases of transfusion-related hemolysis,31 cases of fever and 39 cases of allergic reactions.The adverse reactions of blood transfusion mostly occurred in the recipients who entered red blood cell suspension(31.94%),fresh frozen plasma(26.39%)and platelets(36.11%).Univariate and logistic multivariate analysis showed that the frequency of blood transfusion,history of allergy,history of blood transfusion and primary blood disease were independent risk factors for adverse reactions of blood transfusion(P<0.05).Conclusion:Inputting red blood cell suspension,fresh frozen plasma and platelets can easily cause adverse transfusion reactions,which are often seen in fever and allergic reactions.Active and effective treatment measures and drug treatment can significantly reduce the risk and harm of adverse reactions.
