摘要
目的探讨麻醉深度监测仪辅助下不同浓度七氟醚对老年全麻手术患者血流动力学稳定的影响。方法选取医院2019年8月至2021年9月收治的老年全麻手术患者99例,按双盲法分为0.5最低肺泡有效浓度(MAC)组、0.65 MAC组及1 MAC组,每组各33例。所有患者均予以气管插管诱导麻醉,并在麻醉深度检测仪的辅助下,以不同浓度对3组患者进行七氟醚静吸复合全身麻醉。结果置管后5 min(T1)、切皮时(T2)、术后即刻(T3)时刻,收缩压(SBP)、舒张压(DBP)、血氧饱和度(SpO2)、心率(HR)均较麻醉前(T0)显著降低,且0.5 MAC组显著高于0.65 MAC组及1 MAC组(P<0.05);0.5 MAC组患者睁眼、气管拔管、自主呼吸恢复、运动恢复时间均显著短于0.65 MAC组及1 MAC组(P<0.05)。结论麻醉深度监测仪辅助下0.5 MAC浓度七氟醚对老年全麻手术患者麻醉干预效果显著,可稳定血流动力学指标,改善苏醒相关指标。
Objective To investigate the effect of different doses of sevoflurane on hemodynamic stability in elderly patients undergoing general anesthesia operation with the aid of anesthesia depth monitor.Methods A total of 99 elderly general anesthesia patients admitted to the hospital from August 2019 to September 2021 were divided into 0.5 minimum alveolar effective concentration(MAC)group,0.65 MAC group and 1 MAC group according to double-blind method,with 33 cases in each group.All patients were intubated to induce anesthesia,and with the aid of anesthesia depth detector,sevoflurane intravenous inhalation combined with general anesthesia was performed at different doses in the three groups.Results The systolic blood pressure(SBP),diastolic blood pressure(DBP),blood oxygen saturation(SPO2)and heart rate(HR)of all patients at 5 min after catheterization(T1),at the time of skin incision(T2)and immediately after operation(T3)were significantly lower than those before anesthesia(T0),and those in 0.5 MAC group were significantly higher than those in 0.65MAC group and 1 MAC group(P<0.05).The time of eye opening,tracheal extubation,spontaneous breathing recovery and motor recovery in 0.5 MAC group were significantly shorter than those in 0.65 MAC group and 1 MAC group(P<0.05).Conclusion With the aid of anesthesia depth monitor,sevoflurane with 0.5 MAC concentration has a significant effect on anesthesia intervention in elderly patients undergoing general anesthesia.It can stabilize hemodynamic indexes and improve the related indexes of recovery,which is worthy of clinical promotion.
