SWOT视角下地市级培训基地住院医师规范化培训管理工作的思考与分析——以宜昌市第一人民医院为例

摘要 目的 分析地市级培训基地住院医师规范化培训(简称住培)管理工作现状,深入探讨并提出可行性应对策略与优化方案。方法 运用SWOT方法,分析地市级培训基地住培工作内部环境的优势和劣势,以及外部环境中的机遇和挑战。构建SWOT矩阵,并根据分析结果提出措施和方法。结果 地市级培训基地住培管理工作中,具有地域优势和大学教学资源优势。但是内部也存在对住培工作重视程度不够、指导医师临床带教投入度不够的劣势。在国家一系列政策引导下,应抓住住培管理的机遇,化解省部级医院和本区域内住培工作竞争的威胁。通过构建SWOT矩阵,培训基地应逐步完善住培管理制度,给予全方位支持。住培管理部门应联合其他职能部门,构建全院重视住培的良好氛围。制定指导医师教学激励政策,增加师资教学投入与奉献。不同层级的指导医师及管理者应多途径学习提升,提高管理效能。结论 住培管理中引入SWOT分析方法,构建矩阵模型,能探寻地市级培训基地住培管理工作中的问题,总结住培建设思路和发展方向,值得在住培管理中运用。 Objective To analyze the existing circumstances of standardized residency training in our grade three and first-class hospital of municipal level, in order to provide feasible coping strategies. Methods SWOT analysis was applicated to investigate the strengths, weaknesses, opportunities and threats of standardized residency training in a grade three and first-class hospital. We constructed SWOT matrix, and proposed coping strategies according to the analysis results. Results In the management of standardized residency training in our grade three and first-class hospital of municipal level, we had advantages such as regional advantages and teaching resources of universities. However, there were also disadvantages, such as the importance of standardized residency training by the managers and teachers was insufficient and doctors payed less attention to teaching. Under the guidance of a series of national policies, we should seize the opportunities, and resolve the competitive threats from other hospitals. Through the construction of SWOT matrix, the hospital should establish a perfect construction system and policy support for standardized residency training. It should be managed by multiple departments in the hospital, so as to construct good atmosphere to pay attention to standardized residency training. We shoud formulate incentive policies for teaching, in order to increase teachers investment and dedication. Teachers of different levels and managers should study in many ways and improve management efficiency. Conclusion SWOT analysis may be used in the management of standardized residency training and could find out the problems and shortcomings in order to explore the development direction, which is worthy of application and feedback in standardized residency training management.
