摘要
目的比较双镜联合胆总管切开取石与开放手术治疗肝外胆管结石的临床疗效。方法选取2017年6月-2021年6月我院诊治的62例肝外胆管结石患者,随机分为对照组和观察组,各31例。对照组采用开放手术治疗,观察组采用双镜联合胆总管切开取石治疗,比较两组手术成功率、临床手术指标、术后恢复指标、肝功能指标及并发症发生率。结果观察组手术成功率为93.54%,与对照组的96.77%比较,差异无统计学意义(P>0.05);观察组手术时间、术中出血量、切口长度均小于对照组,胃肠功能恢复时间、首次下床活动时间、住院时间均短于对照组(P<0.05);观察组总胆红素、直接胆红素、降钙素原水平均低于对照组(P<0.05);观察组并发症发生率为6.45%,低于对照组的16.13%(P<0.05)。结论双镜联合胆总管切开取石治疗肝外胆管结石可获得开放手术基本一致的手术成功率,且对手术时间短、术中出血量少、切口小,可减轻对患者的损伤,缩短患者术后恢复时间,降低并发症发生率,对肝功能指标影响较小,值得临床应用。
Objective To compare the clinical efficacy of double-endoscope combined with choledocholithotomy and open surgery in the treatment of extrahepatic bile duct stones.Methods A total of 62 patients with extrahepatic bile duct stones treated in our hospital from June 2017 to June 2021 were randomly divided into control group and observation group,with 31 cases in each group.The control group was treated with open surgery,and the observation group was treated with double endoscopy combined with choledocholithotomy.The success rate,clinical operation index,postoperative recovery index,liver function index and complication rate were compared between the two groups.Results The success rate of operation in the observation group was 93.54%,which was compared with 96.77%in the control group,the difference was not statistically significant(P>0.05).The operation time,intraoperative blood loss and incision length of the observation group were less than those of the control group,and the recovery time of gastrointestinal function,the time of first ambulation and the time of hospitalization were shorter than those of the control group(P<0.05).The levels of total bilirubin,direct bilirubin and procalcitonin in the observation group were lower than those in the control group(P<0.05).The incidence of complications in the observation group was 6.45%,which was lower than 16.13%in the control group(P<0.05).Conclusion Double mirror combined with choledocholithotomy in the treatment of extrahepatic bile duct stones can obtain the same success rate of open surgery,and has short operation time,less intraoperative blood loss and small incision.It can reduce the damage to patients,shorten the recovery time of patients after operation,reduce the incidence of complications,and has little effect on liver function indexes,which is worthy of clinical application.
