脑微出血病人血清β淀粉样蛋白1-42、磷酸化τ-181蛋白的检测意义 被引量:1

Clinical significance of detecting serum Aβ_(1-42) and phosphorylated tau-181 protein in patients with cerebral microbleeds

摘要 目的探讨脑微出血病人血清β淀粉样蛋白1-42(Aβ_(1-42))、磷酸化τ-181蛋白检测的意义。方法收集2019年11月至2020年5月在徐州医科大学附属医院经头颅磁共振成像(MRI)+磁敏感加权成像(SWI)检查证实存在脑微出血的腔隙性脑梗死病人50例为研究组,与之性别、年龄相匹配的不存在脑微出血的腔隙性脑梗死病人50例为对照组。将研究组按照脑微出血的MARS评估量表分为皮层和皮质下组(16例)、深部组(14例)、幕下组(15例)和混合组(5例)。对比各组间血清Aβ_(1-42)、磷酸化τ-181蛋白浓度。结果与对照组(36.63±12.72)ng/L相比,研究组血清Aβ_(1-42)浓度(50.14±25.75)ng/L明显升高(P<0.05);与对照组(17.82±6.93)ng/L相比,研究组血清τ-181浓度(26.40±12.24)ng/L明显升高(P<0.05)。且脑微出血总数目与血清Aβ_(1-42)及血清磷酸化τ-181浓度均呈正相关(r=0.46、0.30,P<0.05)。然而皮层和皮质下组、深部组、幕下组间血清Aβ_(1-42)和磷酸化τ-181浓度差异无统计学意义(P>0.05)。结论脑微出血对认知功能的影响机制与血清Aβ_(1-42)、磷酸化τ-181蛋白的水平呈正相关,临床上对潜在认知功能损害的脑微出血病人检测血清Aβ_(1-42)、磷酸化τ-181蛋白有重要意义。 Objective To investigate the significance of serumβ-amyloid 1-42(Aβ_(1-42))and phosphorylated tau-181 protein detection in patients with cerebral microbleeds(CMBs).Methods A total of 50 patients with lacunar infarction who were confirmed to have cerebral microbleed by cranial magnetic resonance imaging(MRI)+magnetic susceptibility weighted imaging(SWI)at the Affiliated Hospital of Xuzhou Medical University from November 2019 to May 2020 were collected as the research group,and 50 patients with lacunar cerebral infarction who were matched for sex and age without cerebral microbleed were selected as the control group.According to the MARS assessment scale for cerebral micro bleeds,the research group was divided into cortical and subcortical group(16 cases),deep regions(14 cases),infratentorial regions(15 cases),and mixed regions(5 cases).The serum Aβ_(1-42) and phosphorylated tau-181 protein concentrations between the groups were compared.Results The serum Aβ_(1-42) concentration(50.14±25.75)ng/L was significantly higher in the research group than in the control group(36.63±12.72)ng/L(P<0.05);the serum tau-181 concentration(26.40±12.24)ng/L was significantly higher in the research group than in the control group(17.82±6.93)ng/L(P<0.05),and the total number of CMBs was positively correlated with both serum Aβ_(1-42) and serum phosphorylated tau-181 concentrations(r=0.46,0.30,P<0.05).However,the differences in serum Aβ_(1-42) and phosphorylated tau-181 concentrations between the cortical and subcortical groups,the deep group,and the infratentorial regions group were not statistically significant(P>0.05).Conclusion The mechanism of the effect of cerebral microbleeds on cognitive function was positively correlated with serum Aβ_(1-42) and phosphorylated tau-181 protein,and it is clinically important to detect serum Aβ_(1-42) and phosphorylated tau-181 protein in CMBs patients with potential cognitive impairment.
