摘要
目的:研究甲状旁腺增生与腺瘤鉴别诊断中超声弹性成像技术的应用价值。方法:选取2018年6月-2021年6月收治的106例甲状旁腺病变患者(132枚病灶),以术后病理诊断结果分为腺瘤组(54例)与增生组(52例),入组者均行常规超声及超声弹性成像检查,以病理诊断结果为金标准,分析超声检查结果。结果:106例患者(132枚病灶)经手术病理诊断结果为甲状旁腺增生78枚,甲状旁腺腺瘤54枚;两组病灶大小、病灶部位比较,差异无统计学意义(P>0.05);增生组SR值较之腺瘤组更低(P<0.05);SR法对甲状旁腺增生及腺瘤鉴别诊断的准确度、灵敏度及特异度分别为80.30%(106/132)、74.07%(40/54)、84.62%(66/78);评分法对甲状旁腺增生及腺瘤鉴别诊断的诊断准确度、灵敏度及特异度分别为80.30%(106/132)、77.78%(42/54),82.05%(64/78);SR法与评分法的诊断准确性比较,差异无统计学意义(P>0.05)。结论:甲状旁腺增生与腺瘤鉴别诊断中超声弹性成像技术的应用价值较高,SR法、评分法均准确鉴别增生及腺瘤,相比较而言,SR法诊断结果更为客观。
Objective: To study the value of ultrasound elastography in the differential diagnosis of parathyroid hyperplasia and adenoma. Methods: 106 patients with parathyroid diseases(132 lesions) admitted from June 2018 to June 2021 were selected and divided into adenoma group(54 cases) and hyperplasia group(52 cases) according to the postoperative pathological diagnosis results. All the patients were examined with conventional ultrasound and ultrasonic elastography, and the results of ultrasound examination were analyzed based on the pathological diagnosis results. Results: 106 patients(132 lesions) were diagnosed as parathyroid hyperplasia(78 lesions) and parathyroid adenoma(54 lesions) by surgery and pathology;There was no significant difference in lesion size and location between the two groups(P>0.05);SR value in hyperplasia group was lower than that in adenoma group(P<0.05);The accuracy,sensitivity and specificity of SR method in differential diagnosis of parathyroid hyperplasia and adenoma were 80.30%(106/132), 74.07%(40/54) and 84.62%(66/78), respectively;The diagnostic accuracy, sensitivity and specificity of the scoring method in differential diagnosis of parathyroid hyperplasia and adenoma were 80.30%(106/132), 77.78%(42/54) and 82.05%(64/78) respectively;There was no significant difference in the diagnostic accuracy between SR method and scoring method(P>0.05). Conclusion: Ultrasound elastography is of high value in the differential diagnosis of parathyroid hyperplasia and adenoma. SR method and scoring method can accurately differentiate hyperplasia and adenoma. Compared with SR method, SR method is more objective in the diagnosis.
