摘要
目的分析胫骨中下段骨折患者术后出院准备度与其创伤后成长量表(PTGI)评分的关联性,并探讨影响出院准备度的因素。方法选取2019年8月至2021年8月在河南省人民医院就诊的81例胫骨中下段骨折术后患者作为研究对象,通过问卷调查了解其出院准备度、PTGI评分,分析两者关联性,并收集患者基础资料,探讨影响出院准备度的相关因素。结果胫骨中下段骨折术后患者出院准备度量表(RHDS)评分为(140.59±17.39)分,处于中等水平,PTGI评分为(65.34±12.87)分,Spearman分析显示,胫骨中下段骨折术后患者RHDS评分与PTGI评分呈正相关(P<0.05);不同年龄、受教育程度、损伤严重程度评分(ISS)、自觉痛苦程度、合并症种类、居住方式、PTGI评分患者RHDS评分对比,差异有统计学意义(P<0.05);多元回归方程结果显示,年龄、受教育程度、ISS评分、自觉痛苦程度、PTGI评分均为胫骨中下段骨折术后患者RHDS评分的影响因素(P<0.05)。结论胫骨中下段骨折术后患者出院准备度处于中等水平,与创伤后成长具有正相关关系;同时,年龄、受教育程度、ISS评分、自觉痛苦程度等也可对出院准备度产生影响,对此,临床应积极对患者展开出院准备度评估,以实施个性化、针对性干预措施,提高出院指导质量,帮助患者顺利度过康复阶段。
Objective To analyze the association between postoperative readiness for discharge and their posttraumatic growth inventory(PTGI)scores in patients with middle and lower tibial fractures and to explore the factors associated with readiness for discharge.Methods A total of 81 patients with middle and lower tibial fractures who were treated in Henan Provincial People’s Hospital from August 2019 to August 2021 were selected for the study,and their readiness for discharge and PTGI scores were understood by questionnaires,and the correlation between them was analyzed,and the patients’basic data were collected to explore the relevant factors affecting the readiness for discharge.Results The readiness for hospital discharge scale(RHDS)score of patients with middle and lower tibial fractures was(140.59±17.39)points,which was at a moderate level,and the PTGI score was(65.34±12.87)points.Spearman analysis found that the RHDS score of patients after middle and lower tibial fracture was positively correlated with the PTGI score(P<0.05).The RHDS scores of patients with different age,education level,injury severity score(ISS)score,conscious pain level,type of comorbidity,residence mode and PTGI score were statistically significant(P<0.05).The results of multiple regression showed that age,education level,ISS score,conscious pain level and PTGI score were all factors influencing RHDS score of patients after middle and lower tibial fracture(P<0.05).Conclusion Patients’readiness for discharge after middle and lower tibial fracture is at an intermediate level and has a positive correlation with post traumatic growth.Meanwhile,age,education level,ISS score and self-consciousness of pain can also have an impact on readiness for discharge,for which clinical patients should be actively assessed for readiness for discharge to implement personalized and targeted interventions to improve the quality of discharge guidance and help patients pass through the rehabilitation phase smoothly.
