摘要
目的探讨分析谷氨酰胺和生长激素强化营养对重症肺结核患者免疫功能的影响。方法将78例重症肺结核患者随机分成A、B、C组,每组各26例,A组予以标准营养支持治疗,B组予以标准营养支持^(+)谷氨酰胺治疗,C组标准营养支持^(+)谷氨酰胺和生长激素治疗,比较三组干预前后患者营养指标、免疫功能变化情况。结果营养干预后,C组的红细胞计数、血清白蛋白、血清前白蛋白水平显著高于A组、B组(P<0.05)。C组患者的CD3^(+)、CD4^(+)以及CD4^(+)/CD8^(+)水平明显大于A组、B组,且CD8^(+)水平显著少于A组、B组(P<0.05)。三组痰菌转阴率比较无明显的差异(P>0.05),但C组影像学总疗效、临床总有效率显著高于B组、A组(P<0.05)。结论谷氨酰胺和生长激素强化营养能够显著改善重症肺结核患者的营养状况和免疫功能,利于提高治疗效果,促进预后的改善。
Objective To explore the effect of glutamine and growth hormone on the immune function of patients with severe pulmonary tuberculosis.Methods Seventy-eight patients with severe pulmonary tuberculosis were randomly divided into three groups,26 cases in each group.Group A received standard nutrition support,group B received standard nutrition support and glutamine treatment,group C received standard nutrition support and glutamine and growth hormone treatment.The nutritional index,immune function and quality of life of the three groups were compared.Results After nutrition intervention,the levels of RBC,serum albumin and serum prealbumin in group C were significantly higher than group A and group B(P<0.05).The levels of CD3^(+),CD4^(+)and CD4^(+)/CD8^(+)in group C were significantly higher than group A and B,and the levels of CD8^(+)were significantly lower than group A and B(P<0.05).There was no significant difference in the negative rate of sputum bacteria among the three groups(P>0.05),but the total imaging efficacy and clinical response rate of group C were significantly higher than the group B and group A(P<0.05).Conclusion Glutamine and growth hormone fortified nutrition can significantly improve the nutritional status and immune function of patients with severe pulmonary tuberculosis,improve the treatment effect and promote the improvement of prognosis.
