摘要
目的研究B超引导下腰丛复合坐骨神经阻滞麻醉对老年股骨粗隆间骨折患者麻醉效果及免疫功能的影响。方法从2019年2月至2019年12月我院收治的老年股骨粗隆间骨折患者中,随机选取120例患者作为研究对象。他们被随机分为研究组(60例)和参照组(60例)。两组均采取人工股骨头置换术治疗。参照组采用蛛网膜下腔阻滞联合硬膜外麻醉。研究组在B超引导下采用腰丛联合坐骨神经阻滞麻醉。比较两组麻醉效果及免疫功能。结果研究组神经阻滞起效时间低于参照组,差异具有统计学意义(P<0.05)。研究组神经阻滞维持时间长于参照组,差异具有统计学意义(P<0.05)。研究组不良反应发生率低于参照组,差异有统计学意义(P<0.05)。结论B超引导下腰丛复合坐骨神经阻滞对老年股骨粗隆间骨折患者有更显著的麻醉效果,会提高患者的免疫功能,提高手术后的生活质量,减少康复时间,有着更好的推广价值。
Objective To study the effect of anesthesia combined with sciatic nerve block guided by ultrasound on the anesthesia effect and immune function of elderly patients with intertrochanteric fractures.Methods From the elderly patients with intertrochanteric fractures treated in our hospital from February 2019 to December 2019,120 patients were randomly selected as the research object of the experiment,and randomly divided into 60 cases in the study group and 60 cases in the reference group.Both groups were treated with artificial femoral head replacement.The reference group was treated with subarachnoid block anesthesia combined with epidural anesthesia.The study group was anesthetized by B-ultrasound-guided lumbar plexus combined with sciatic nerve block.The effects of anesthesia and immune function were compared between the two groups.Results The onset time of nerve block in the study group was lower than that in the reference group,and the difference was statistically significant(P<0.05).The nerve block maintenance time of the study group was longer than that of the reference group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in the study group was lower than that in the reference group,and the difference was statistically significant(P<0.05).Conclusion B-ultrasound-guided lumbar plexus compound sciatic nerve block has a more significant anesthesia effect on elderly patients with intertrochanteric fractures,it will improve the patient's immune function,improve the quality of life after surgery,reduce the recovery time,and have a better promotion value.
