摘要
慢性萎缩性胃炎(CAG)为消化系统常见病与疑难病。周斌教授临证诊疗CAG多年,经验丰富,对CAG病机动态演绎发展规律进行了系统归纳总结,认为CAG多肇始于脾胃虚弱,脾胃虚弱可招木乘、可致痰阻、可成络瘀、可损肾阳、可为毒蕴,如此层层深入递进,终以癌变。周斌教授承整体恒动的预防性思维,应用“先安未受邪之地”学说辨治该病,临证及早对“未受邪之地”施以药物顾护,在健运脾胃的基础上酌加以调畅木性、化痰通阻、活血通络、温补肾阳、祛邪解毒之品,药先于证而用,以阻其病理损害之径,而达“先安”。笔者通过对周斌教授应用“先安未受邪之地”学说治疗CAG经验进行探析,附加验案举隅,期为临证提供参佐。
Chronic atrophic gastritis(CAG) is a common and difficult disease of the digestive system. Professor ZHOU Bin has many years of clinical diagnosis and treatment of CAG and has rich experience. He systematically summarizes the dynamic deduction and development of CAG pathogenesis. He believes that CAG mostly originates from the weakness of the spleen and stomach, which can lead to the growth of wood, cause phlegm resistance, form blood stasis, damage the kidney yang,and poison accumulation. Such layers of deep progressive, eventually to cancer. Professor ZHOU Bin adhered to the preventive thinking of the whole and constant movement, applied the theory of ‘caring the healthy part firstly’ to differentiate and treat the disease, and applied medicine care to the ‘healthy part’ as soon as possible in the clinical treatment. On the basis of strengthening the spleen and stomach, it is recommended to regulate the nature of wood, resolve phlegm and block resistance, activate blood and relieve collaterals, warm and invigorate kidney yang, remove pathogens and detoxify. Medicine is used before the syndrome to prevent the pathologic damage and achieve ‘first safety’. The author explores and analyzes professor ZHOU Bin’s experience in treating CAG by applying the theory of ‘caring the healthy part firstly’, and adds some test cases, hoping to provide reference for clinical evidence.
