妊娠期高血压患者尿转铁蛋白和尿微量白蛋白含量变化及其与糖脂代谢的相关性研究 被引量:3

摘要 目的研究妊娠期高血压患者尿转铁蛋白和微量白蛋白与健康孕妇的差异,并探讨妊娠期高血压患者转铁蛋白和微量白蛋白与糖脂代谢的相关性。方法回顾性选取2021年1~12月在三二〇一医院进行产检的妊娠期高血压患者60例和健康孕妇60例作为研究对象。比较两组孕妇尿转铁蛋白、尿微量白蛋白、空腹血糖(FBG)、餐后2 h血糖(2 h PG)、糖化血红蛋白(HbA1c)、空腹胰岛素(Fins)、胰岛素抵抗指数(HOMA-IR)、总胆固醇、甘油三脂、高密度胆固醇(HDL)和低密度胆固醇(LDL),并分析相关性。结果妊娠期高血压孕妇尿转铁蛋白和微量白蛋白为(5.20±0.76)、(16.33±2.20)mg/L,均显著高于健康孕妇[(1.66±0.22)、(6.48±0.75)mg/L],差异均有统计学意义(P<0.05)。妊娠期高血压孕妇Fins和HOMA-IR为(16.77±3.97)uIU/mL、3.69±1.25,均显著高于健康孕妇[(9.39±3.63)uIU/mL、1.94±0.96],差异均有统计学意义(P<0.05),而FBG、2 h FG和HbA1c与健康孕妇相比,差异均无统计学意义(P>0.05)。妊娠期高血压孕妇甘油三酯、LDL和总胆固醇为(2.28±0.35)、(4.50±1.19)、(5.63±0.80)mmol/L,均高于健康孕妇[(1.07±0.12)、(2.75±0.89)、(3.62±0.41)mmol/L],差异均有统计学意义(P<0.05),而HDL与健康孕妇相比差异均无统计学意义(P>0.05)。妊娠期高血压孕妇尿转铁蛋白和尿微量白蛋白均与Fins、HOMA-IR、甘油三酯和总胆固醇呈显著正相关(P<0.05),而与FBG、2 h PG、HbA1c、HDL和LDL均无相关性(P>0.05)。结论妊娠期高血压孕妇尿转铁蛋白和微量白蛋白含量升高,并对妊娠期高血压孕妇糖脂代谢产生影响。 Objective To study the differences of urinary transferrin and microalbumin in patients with gestational hypertension(GH)and healthy pregnant women,and to explore the correlation between transferrin and microalbumin in GH patients with glucose and lipid metabolism.Methods A total of 60 GH patients and 60 healthy pregnant women who underwent obstetric examination in 3201 Hospital from January to December 2021 were selected as the research subjects.Urinary transferrin,microalbumin,fasting blood glucose(FBG),2-hour postprandial blood glucose(2 h PG),glycated hemoglobin(HbA1c),fasting insulin(Fins),insulin resistance index(HOMA-IR),total cholesterol(TC),triglycerides(TG),high-density cholesterol(HDL),and low-density cholesterol(LDL)of the two groups of pregnant women were compared,and analyzed the correlation.Results Urinary transferrin and microalbumin in GH pregnant women were(5.20±0.76),(16.33±2.20)mg/L,which were significantly higher than those in healthy pregnant women[(1.66±0.22),(6.48±0.75)mg/L],the differences were statistically significant(P<0.05).Fins and HOMA-IR of GH pregnant women were(16.77±3.97)uIU/mL,3.69±1.25 significantly higher than those of healthy pregnant women[(9.39±3.63)uIU/mL,1.94±0.96],the differences were statistically significant(P<0.05),while FBG,2 h FG and HbA1C had no significant difference compared with healthy pregnant women(P>0.05).TG,LDL and TC of GH pregnant women were higher(2.28±0.35),(4.50±1.19),(5.63±0.80)mmol/L,than those of healthy pregnant women[(1.07±0.12),(2.75±0.89),(3.62±0.41)mmol/L],the differences were statistically significant(P<0.05),while there was no significant difference in HDL compared with healthy pregnant women(P>0.05).Urinary transferrin and urinary microalbumin in pregnant women with GH were significantly positively correlated with Fins,HOMA-IR,TG and TC(P<0.05),but had no correlation with FBG,2 h PG,HbA1c,HDL and LDL(P<0.05).Conclusion Urinary transferrin and microalbumin levels are elevated in pregnant women with gestational hypertension and affect glucose and lipid metabolism in pregnant women with GH.
