异丙酚与右美托咪定在脑胶质瘤切除术中的脑保护效应对比 被引量:1

摘要 目的 对比应用不同麻醉药物在脑胶质瘤切除术中对脑部的保护作用。方法 选取濮阳市人民医院2015年6月至2018年5月期间接收的60例脑胶质瘤患者,按摸球法分为两组,对照组、研究组在脑胶质瘤切除术中分别使用异丙酚、右美托咪定两种麻醉药物。比较两组患者术后临床相关指标;比较两组不同时间点的血清相关指标(S100β蛋白、神经元特异性烯醇化酶NSE)变化。结果 研究组插管时间、苏醒时间短于对照组,差异有统计学意义(P <0.05);研究组肿瘤切除时、手术完成时、术后24 h后血清S100β蛋白、NSE水平与对照组相比较低,差异有统计学意义(P <0.05)。两组麻醉后血清丙二醛(MDA)水平明显比麻醉诱导前升高,研究组麻醉后三个时间点血清SOD水平均低于对照组,差异有统计学意义(P <0.05);两组麻醉后血清超氧化物歧化酶(SOD)水平明显比麻醉诱导前降低,研究组麻醉后三个时间点SOD水平均高于对照组,差异有统计学意义(P <0.05)。结论 在脑胶质瘤切除术中应用右美托咪定,能够减轻氧化应激反应,对脑保护效应更为显著,可有效减少脑组织损伤。 Objective To compare the protective effect of different anesthetic drugs on the brain in glioma resection.Methods Sixty patients with glioma who were admitted to Puyang People's Hospital from June 2015 to May 2018 were divided into two groups according to the ball-touch method.Propofol and dexmedetomidine were respectively used in the control group and study group during glioma resection.Postoperative clinical indicators were compared between the two groups.The changes of serum-related indexes(S100βprotein,neuron-specific enolase NSE)were compared between the two groups at different time points.Results The intubation time and recovery time of the study group were shorter than those of the control group,and the difference was statistically significant(P<0.05).The levels of serum S100βprotein and NSE in the study group were lower than those in the control group at the time of tumor resection,the completion of the surgery,and 24 h after surgery,and the difference was statistically significant(P<0.05).The serum malondialdehyde(MDA)level of the two groups after anesthesia was significantly higher than that before anesthesia induction,and the serum SOD level of the study group was lower than that of the control group at three-time points after anesthesia,the difference was statistically significant(P<0.05).The serum superoxide dismutase(SOD)level of the two groups after anesthesia was significantly lower than that before anesthesia induction,and the SOD level of the study group was higher than that of the control group at three-time points after anesthesia,the difference was statistically significant(P<0.05).Conclusion The application of dexmedetomidine in glioma resection can reduce the oxidative stress response,and has a more significant protective effect on the brain,which can effectively reduce the damage to brain tissue.
