摘要
目的 研究国家标准化代谢性疾病管理中心(MMC)模式对老年2型糖尿病患者用药依从性及血糖水平的影响。方法 收集2016年7月至2019年7月在江苏省泰州市中西医结合医院接受治疗的100例2型糖尿病患者纳入试验,通过抽签方式随机分成观察组和对照组,各50例,对照组采取常规干预模式,观察组在对照组的基础上采用MMC模式对患者进行管理;比较两组患者干预前后不同时间用药依从性;比较两组患者干预前后2型糖尿病自我管理行为问卷(2-DSCS)、身体质量指数(BMI)、腰围(WC)、空腹血糖(FBG)及糖化血红蛋白(HbA1c)水平;另比较两组干预期间不良反应发生情况。结果 两组干预前及干预1周后用药依从性好的患者比例无显著差异(P> 0.05);干预1个月后,两组依从性好的患者比例均高于干预前,其中观察组依从性好的患者比例高于本组干预1周后及同时段对照组,差异有统计学意义(P <0.05)。两组干预前自我管理行为评分比较,差异无统计学意义(P> 0.05)。干预后,两组自我管理行为评分均高于干预前,且观察组高于对照组,差异有统计学意义(P <0.05)。观察组干预后BMI、WC、FBG、HbA1c显著低于干预前及对照组干预后,差异有统计学意义(P <0.05)。观察组不良反应发生率为16.00%,与对照组(22.00%)比较,差异无统计学意义(P> 0.05)。结论 MMC模式能有效提高2型糖尿病患者用药依从性,降低患者血糖水平,增强患者自我管理能力,安全性良好,临床价值较高。
Objective To investigate the effects of the national metabolic management center(MMC)model on medication compliance and blood glucose level in elderly patients with type 2 diabetes.Methods A total of 100 patients with type 2 diabetes who received treatment at Taizhou Hospital of Integrated Traditional and Western Medicine in Jiangsu Province from July 2016 to July 2019 were included in the study and randomly divided into an observation group and a control group by lottery,with 50 cases in each group.The control group received the conventional intervention mode,and the observation group received the MMC mode based on the control group.The medication compliance before and after the intervention was compared between the two groups.The levels of type 2 diabetes self-care scale(2-DSCS),body mass index(BMI),waist circumference(WC),fasting blood glucose(FBG),and glycosylated hemoglobin(HbA1c)before and after intervention were compared between the two groups.The occurrence of adverse reactions during the intervention was also compared between the two groups.Results There was no significant difference in the proportion of patients with good medication compliance between the two groups before intervention and 1 week after intervention(P>0.05).After 1 month of intervention,the proportion of patients with good compliance in both groups was higher than before intervention,and the proportion of patients with good compliance in the observation group was higher than that in the control group after 1 week of intervention and at the same period,the difference was statistically significant(P<0.05).There was no significant difference in the scores of self-management behavior between the two groups before intervention(P>0.05).After the intervention,the scores of self-management behavior in both groups were higher than before the intervention,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).BMI,WC,FBG,and HbA1c in the observation group after intervention were significantly lower than before intervention and after intervention in the control group,with statistical significance(P<0.05).The incidence of adverse reactions in the observation group was 16.00%,and there was no significant difference between the observation group and the control group(22.00%)(P>0.05).Conclusion MMC model can effectively improve the medication compliance of type 2 diabetes patients,reduce the blood sugar level of patients,and enhance the self-management ability of patients,with good safety and high clinical value.
