摘要
目的:分析超声弹性成像(ultrasonic elastography,UE)在甲状腺结节(thyroid nodule,TN)诊断中的应用价值。方法:回顾性分析2020年1月—2022年1月于成都市新都区中医医院就诊的80例TN患者的临床资料,所有患者术前均完成常规超声(coventional ultrasonogaphy,CUS)单独诊断以及联合UE诊断,以手术和超声引导下穿刺活检的病理结果为金标准,分析UE在TN患者良、恶性中的鉴别诊断价值。结果:术后病理活检结果显示,80例TN患者中55例良性结节,25例恶性结节。CUS检查结果显示,良、恶性TN患者的结节形态、边界、纵横比、内部回声信号、钙化特征检出情况比较差异均有统计学意义(P<0.05)。良、恶性TN患者UE评分差异有统计学意义(P<0.05)。CUS检出恶性结节30例、良性结节50例,其中6例恶性结节漏诊,11例良性结节误诊。CUS联合UE检出恶性结节27例、良性结节53例,其中1例恶性结节漏诊,3例良性结节误诊。CUS联合UE对TN良、恶性的鉴别诊断灵敏度、特异度、准确率、阳性预测值、阴性预测值均显著高于CUS诊断(P<0.05)。结论:UE联合CUS可进一步提高TN良、恶性的鉴别诊断效能,具有临床应用价值。
Objective To analyze the application value of ultrasound elastography(UE)in the diagnosis of thyroid nodule(TN).Methods The clinical data of 80 patients with TN who were treated in Chengdu Xindu District Hospital of Traditional Chinese Medicine from January 2020 to January 2022 were retrospectively analyzed.All patients completed conventional ultrasonography(CUS)diagnosis alone and combined UE diagnosis before operation.With the pathological results of surgery and ultrasoundguided needle biopsy as the gold standard,the differential diagnosis value of UE in benign and malignant TN patients was analyzed.Results Postoperative pathological biopsy results showed that among the 80 patients with TN,there were 55 benign nodules and 25 malignant nodules.The results of CUS examination showed that the nodule morphology,boundary,aspect ratio,internal echo signal,and calcification features were significantly different between benign and malignant TN patients(P<0.05).There was a statistically significant difference in UE score between benign and malignant TN patients(P<0.05).CUS detected 30 malignant nodules and 50 benign nodules,of which 6 malignant nodules were missed and 11 benign nodules were misdiagnosed.CUS combined with UE detected 27 malignant nodules and 53 benign nodules,of which 1 malignant nodule was missed and 3 benign nodules were misdiagnosed.The sensitivity,specificity,accuracy,positive predictive value and negative predictive value of the differential diagnosis of malignancy were significantly higher than those of CUS(P<0.05).Conclusion UE combined with CUS can further improve the differential diagnosis efficiency of benign and malignant TN,which has clinical application value.
