右美托咪定对肝移植大鼠心肌损伤的保护作用 被引量:1

Protective effects of dexmedetomidine on myocardial injury in liver transplant rats

摘要 目的探讨右美托咪定对肝移植患者心肌损伤的保护作用。方法取SD大鼠30只,用随机数字表法均分为3组(n=10),假手术组(S组)仅开关腹、游离相关血管,模型组(M组)制备大鼠自体肝移植模型,右美托咪定组(D组)造模前30 min腹腔注射右美托咪定50μg/kg。3组开放肝循环后6 h,经大鼠肝下下腔静脉采集静脉血,取血后处死大鼠,并取心肌组织。采用独立样本F检验比较3组的血清相关炎性因子和心肌损伤标志物,两组间比较采用SNK的q检验。结果M、D组大鼠血清肿瘤坏死因子-α(TNF-α)、白细胞介素(IL)-6、高迁移率族蛋白B1(HMGB1)及肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK-MB)水平均高于S组(F=340.741,P<0.05),但D组低于M组(F=88.102,P<0.05);S组大鼠心肌组织病理变化明显优于M组,D组病理变化轻于M组;M、D组大鼠心肌超氧化物歧化酶(SOD)活性、丙二醛(MDA)水平以及心肌细胞凋亡指数均高于S组(F=23.948,P<0.05),但D组均低于S组(F=67.241,P<0.05)。结论肝移植大鼠应用右美托咪定可有效抑制模型大鼠体内炎性反应,对心肌损伤具有一定的保护作用。 Objective To explore the protective effect of dexmedetomidine on myocardial injury in liver transplant rats.Methods For animal experiment,30 SD rats were randomly divided into three groups(n=10).The abdomens were opened,the related vessels were freed and the cuts were fixed in the sham operation group(group S).In the model group(group M)rat autologous liver transplantation models were prepared.The rats in dexmedetomidine group(group D)were intraperitoneally injected with 50μg/kg dexmedetomidine 30 min before modeling.Venous blood was collected through the inferior vena cava of the liver 6 h after reperfusion.After blood collection,the rats were killed and the myocardial tissues were taken out.The serum related inflammatory factors and myocardial injury indexes of the three groups were compared using F-test for multiple groups of independent sample and Student-Newman-Keuls(SNK)-q test between two groups.Results Tumor necrosis factor-α(TNF-α),interleukin(IL)-6,high mobility group protein B1(HMGB1),cardiac troponin I(cTnI)and creatine kinase-MB(CK-MB)in group M were higher than those in group S(F=340.741,P<0.05),and those in group D were lower than in group M(F=88.102,P<0.05).The pathologic changes of myocardial tissue in group S were significantly milder than those in group M,and those in the group D were milder than in group M.The myocardial superoxide dismutase(SOD)activity,malondialdehyde,malonic dialdehyde(MDA)level and cardiomyocyte apoptosis index in group M and group D were higher than those in group S(F=23.948,P<0.05),and those in group D were lower than those in group S(F=67.241,P<0.05).Conclusion Administration of dexmedetomidine during liver transplantation can effectively inhibit the inflammatory response and protect the myocardial injury in liver transplantation rats.
