纤维支气管镜肺泡灌洗联合高流量吸氧湿化仪治疗重症肺炎伴呼吸衰竭患者的临床研究 被引量:5

摘要 目的:探讨纤支镜肺泡灌洗联合高流量吸氧湿化仪治疗重症肺炎伴呼吸衰竭患者的效果.方法:将2019年1月至2020年12月息县中心医院重症医学科收治的82例重症肺炎伴呼吸衰竭患者纳入研究.将纤支镜肺泡灌洗联合高流量吸氧湿化仪治疗患者纳入观察组,将高流量吸氧湿化仪治疗患者纳入对照组,各41例.对比两组临床疗效,生命体征指标(心率、呼吸频率)及血气分析指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))].结果:观察组总有效率高于对照组;与治疗前比较,两组治疗1周后心率、呼吸频率降低,且观察组降幅更大;两组治疗1周后PaO_(2)升高,PaCO_(2)下降,且观察组更显著,差异均有统计学意义(P<0.05).结论:纤支镜肺泡灌洗联合高流量吸氧湿化仪治疗重症肺炎伴呼吸衰竭患者疗效更佳,可改善生命体征及血气分析指标. Objective:To investigate efects of bronchoalveolar lavage by fiberoptic bronchoscope combined with high flow oxygen humidification instrument in treating severe pneumonia with respiratory failure.Methods:Eighty-two patients with severe pneumonia with respiratory failure treated in Department of Critical Medicine,Xi County Central Hospital from January 2019 to December 2020 were included in the study.Patients treated with bronchoalveolar lavage by fiberoptic bronchoscope combined with high flow oxygen humidification instrument were included in observation group,and patients treated by high flow oxygen humidification instrument were included in control group,with 41 cases in each.The clinical efficacy,vital signs(heart rate,respiratory rate)and blood gas analysis indexes[arterial partial pressure of oxygen(PaO_(2))and arterial partial pressure of carbon dioxide(PaCO_(2))]of both groups were compared.Results:The total effective rate of observation group was higher than that of control group.Compared with before treatment,the heart rate and respiratory rate of two groups were decreased one week after treatment,and the decrease was greater in the observation group.PaO_(2) was increased and PaCO_(2) was decreased in both groups one week after treatment,and the observation group was more significant,with statistical significant differences(P<0.05).Conclusion:The combined regimen has better efficacy and can improve the vital signs and blood gas analysis indexes.
