摘要
目的分析急性冠状动脉综合征(ACS)患者术前血清脑钠肽(BNP)、心肌肌钙蛋白I(cTnI)水平与经皮冠状动脉介入(PCI)术后严重心律失常发生的相关性。方法选择241例ACS PCI患者,根据是否发生严重心律失常分为发生组与未发生组。比较两组术前血清BNP、cTnI水平和术后严重心律失常发生率。采用Logistic回归分析BNP及cTnI与严重心律失常的相关性。结果发生组左心室舒张期末内径(LVEDD)、BNP、cTnI水平均高于未发生组,左心室射血分数(LVEF)低于未发生组(P<0.05)。LVEDD、BNP、cTnI是PCI术后严重心律失常发生的影响因素(P<0.05),LVEF是保护因素(P<0.05);BNP、cTnI与LVEDD呈正相关(P<0.05),与LVEF呈负相关(P<0.05)。BNP、cTnI联合检测对PCI术后严重心律失常预测的诊断效能最高。结论血清BNP、cTnI水平与ACS患者PCI术后严重心律失常的发生密切相关。
Aim To analyze the correlation between the levels of preoperative serum brain natriuretic peptide(BNP)and cardiac troponin I(cTnI)before treatment in patients with acute coronary syndrome(ACS)and the occurrence of severe arrhythmia after percutaneous coronary intervention(PCI).Methods 241 cases of ACS PCI patients in our hospital were selected and divided into the occurrence group and the non-occurrence group according to whether serious arrhythmia occurred.The levels of serum BNP and cTnI before operation and the incidence of serious arrhythmia after operation were compared between the two groups.Logistic regression was used to analyze the correlation between serum BNP,cTnI and severe arrhythmia.Results The levels of left ventricular end-diastolic diameter(LVEDD),serum BNP and cTnI in severe arrhythmia group were higher than those in non-severe arrhythmia group,ventricular ejection fraction(LVEF)was lower than that in non-severe arrhythmia group(P<0.05).LVEDD,BNP,cTnI were the influencing factors of severe arrhythmia after PCI(P<0.05),and LVEF was the protective factor(P<0.05).Serum BNP and cTnI levels were positively correlated with LVEDD(P<0.05),and negatively correlated with LVEF(P<0.05).The combined detection of BNP and cTnI has the highest diagnostic efficacy in predicting severe arrhythmia after PCI.Conclusion Serum BNP and cTnI levels are closely related to the occurrence of severe arrhythmia in ACS patients after PCI.
