摘要
目的 探讨妊娠期高血压产妇应用彩色多普勒超声对宫内窘迫的诊断价值。方法 将2019年1月—2020年1月在株洲市中心医院诊断明确的44例妊娠期高血压伴宫内窘迫患者纳入观察组,同时选择40例同期检查的正常孕妇作为对照组。两组研究对象均在孕30周前和分娩前接受彩色多普勒超声检查,比较两组研究对象不同孕周时的胎儿脐动脉(umbilical artery,UA)、大脑中动脉(middle cerebral artery,MCA)的收缩期最大流速(peak velocity during ventricular systole,S)与舒张末期流速(peak velocity during ventricular diastole,D)的比值(S/D)、搏动指数(pulsatility index,PI)和阻力指数(resistance index,RI)指标水平变化情况。结果 对照组随孕周增加,UA和MCA的S/D、PI和RI值均呈现下降趋势,而观察组的UA血流动力学指标呈现明显升高趋势,MCA血流动力学指标呈现明显降低趋势,上述差异均有统计学意义(P <0.05)。结论 妊娠期高血压产妇应用彩色多普勒超声检查能够对胎儿的脐动脉、大脑中动脉血流情况进行有效的监测和评估,能够准确提高对宫内窘迫的预测和评估。
Objective To explore the value of color Doppler ultrasound in the diagnosis of intrauterine distress in pregnant women with gestational hypertension.Methods A total of 44 patients with gestational hypertension and intrauterine distress diagnosed in Zhuzhou Central Hospital from January 2019 to January 2020 were included in the observation group,and 40 normal pregnant women who were examined during the same period were selected as the control group.All subjects in the two groups underwent color Doppler ultrasound examination before 30 weeks of gestation and delivery.Changes in the ratio(S/D),pulsatility index(PI) and resistance index(RI) of the peak velocity during systolic systole(S) to the peak velocity during diastolic diastole(D) of the umbilical artery(UA) and middle cerebral artery(MCA) of the two groups at different gestational weeks were compared.Results With the increase of gestational weeks,the S/D,PI and RI values of UA and MCA in the control group showed a downward trend,while the UA hemodynamic indexes in the observation group showed a significant upward trend,while the MCA hemodynamic indexes showed a significant downward trend,and the above differences were statistically significant(P <0.05).Conclusion The application of color Doppler ultrasound in pregnant women with PIH can effectively monitor and evaluate the fetal umbilical artery and middle cerebral artery blood flow,and can accurately improve the prediction and evaluation of intrauterine distress.
