ICAM-1、8-iso-PGF2α在急性缺血性脑卒中患者中的表达及与认知功能的关系 被引量:4

摘要 目的探讨细胞间黏附分子-1(ICAM-1)、8异构前列腺异素(8-iso-PGF2α)在急性缺血性脑卒中患者中的表达及与认知功能的关系。方法选取2019年11月至2021年11月在周口市第一人民医院进行治疗的急性缺血性脑卒中患者90例,按照是否存在认知功能障碍将患者分为对照组和研究组各45例。对两组患者一般资料、ICAM-1、8-iso-PGF2α及认知功能进行检测对比。根据美国国立卫生院神经功能缺损(NIHSS)评分标准将患者分为轻度、中度、重度,检测不同严重程度的ICAM-1、8-iso-PGF2α及认知功能。分析ICAM-1、8-iso-PGF2α与认知功能之间的相关性。采用Logistic回归分析影响患者急性缺血性脑卒中患者认知功能的危险因素;受试者工作曲线(ROC)分析ICAM-1、8-iso-PGF2α对急性缺血性脑卒中患者认知功能的诊断价值。结果两组患者脑卒中严重程度、梗死位置、NIHSS评分差异均有统计学意义(P<0.05)。研究组ICAM-1、8-iso-PGF2α水平较高,NIHSS评分较低(P<0.05)。重度ICAM-1、8-iso-PGF2α水平均高于轻度、中度,认知功能低于轻度、中度(P<0.05)。单因素Logistic回归分析,结果示,急性缺血性脑卒中发生均与脑卒中严重程度、梗死位置、NIHSS评分、高血压、糖尿病、ICAM-1、8-iso-PGF2α水平相关,属独立因素(P<0.05)。Pearson检验结果示,ICAM-1、8-iso-PGF2α水平与认知功能呈负相关(r=0.321,P=0.031;r=0.349,P=0.019)。ROC曲线显示,两项联合对急性缺血性脑卒中认知功能的预测价值较高(95%CI:0.828,1.000)特异度和敏感度分别为:90.91%、84.62%。结论ICAM-1、8-iso-PGF2α水平与急性缺血性脑卒中患者认知功能有关,且两者联合检测对急性缺血性脑卒中患者是否认知功能障碍预测价值较高。 Objective To investigate the expression of intercellular adhesion molecule-1(ICAM-1)and 8 isomeric prostagisisin(8-iso-pgF2α)in patients with acute ischemic stroke and their relationship with cognitive function.Methods A total of 90 patients with acute ischemic stroke who received treatment in Zhoukou First People's Hospital from November 2019 to November 2021 were selected and divided into the control group and the study group with 45 patients in each group according to whether they had cognitive dysfunction.The general data,ICAM-1,8-iso-PGF2αand cognitive function of the two groups were compared.According to the NIHSS scoring standard,the patients were divided into mild,moderate and severe levels,and ICAM-1,8-iso-PGF2αand cognitive function of different severity were detected.The correlation between ICAM-1,8-iso-PGF2αand cognitive function was analyzed.Logistic regression was used to analyze the risk factors affecting cognitive function in patients with acute ischemic stroke.ROC curve was adopted to analyze the diagnostic value of ICAM-1,8-iso-PGF2αfor cognitive function in patients with acute ischemic stroke.Results There were significant differences in stroke severity,infarct location and NIHSS score between 2 groups(P<0.05).The levels of ICAM-1 and 8-iso-PGF2αwere higher in the study group,and the NIHSS score was lower(P<0.05).The levels of ICAM-1 and 8-iso-PGF2αin severe patients were higher than those in mild and moderate patients,and the cognitive function was lower than that in mild and moderate patients(P<0.05).Univariate Logistic regression analysis showed that the occurrence of acute ischemic stroke was correlated with stroke severity,infarct location,NIHSS score,hypertension,diabetes,ICAM-1 and 8-iso-PGF2αlevels,which were independent factors(P<0.05).Pearson test showed that ICAM-1 and 8-iso-PGF2αlevels were negatively correlated with cognitive function(r=0.321,P=0.031;r=0.349,P=0.019).ROC curve showed that the combination of the two items had a high predictive value for cognitive function in acute ischemic stroke(95%CI:0.828,1.000).The specificity and sensitivity were 90.91%and 84.62%,respectively.Conclusion ICAM-1 and 8-iso-PGF2αlevels are associated with cognitive function in patients with acute ischemic stroke,and the combined detection of ICAM-1 and 8-iso-PGF2αhas high predictive value for cognitive dysfunction in patients with acute ischemic stroke.
