摘要
目的探讨微生态制剂对进展期胃癌术后化疗患者化疗不良反应及肠道菌群的影响,为该类患者的治疗提供参考。方法选取2019年1月至2021年1月我院收治的96例进展期胃癌术后化疗患者作为研究对象,采用随机数字表法分为观察组及对照组,各48例。对照组患者予以肠内营养治疗,观察组在此基础上予以微生态制剂治疗。比较两组患者化疗不良反应发生情况,治疗前后肠道菌群、免疫功能及营养状态。结果观察组患者腹泻、食欲降低、恶心呕吐、消瘦等化疗不良反应发生率分别为14.58%、12.50%、27.08%、18.75%,均低于对照组的35.42%、37.50%、47.92%、37.50%,差异有统计学意义(均P<0.05)。观察组患者治疗后肠道志贺菌、沙门菌数量均低于对照组,差异有统计学意义(均P<0.05)。观察组患者治疗后CD3+、CD4+细胞及CD4+/CD8+比例均高于对照组,差异有统计学意义(均P<0.05)。观察组患者治疗后转铁蛋白(TRF)、白蛋白(ALB)、前白蛋白(PA)水平均高于对照组,差异有统计学意义(均P<0.05)。结论微生态制剂能显著减少进展期胃癌术后化疗患者的化疗不良反应,维持肠道菌群稳定,提高免疫能力,改善营养状态。
Objective To explore the effects of probiotics on chemotherapy side effects and intestinal floras in patients after chemotherapy for advanced gastric cancer so as to provide reference for their treatment.Methods A total of 96 patients after chemotherapy for advanced gastric cancer in the hospital were enrolled as the research objects between January 2019 and January 2021.According to random number table method,they were divided into observation group and control group,48 cases in each group.The control group was treated with enteral nutrition,while observation group was additionally given probiotics.The occurrence of chemotherapy side effects,intestinal floras,immune function and nutritional status before and after treatment were compared between the two groups.Results The incidence rates of chemotherapy side effects such as diarrhea,anorexia,nausea and vomiting,and emaciation in observation group were 14.58%,12.50%,27.08%and 18.75%,lower than those in control group(35.42%,37.50%,47.92%,37.50%),respectively.After treatment,counts of Shigella and Salmonella in observation group were lower than those in control group(all P<0.05),CD3+,CD4*and CD4+/CD8+ratio were higher than those in control group(all P<0.05),and levels of transferrin(TRF),albumin(ALB)and prealbumin(PA)were higher than those in control group(all P<0.05).Conclusion Probiotics can significantly reduce chemotherapy side effects in patients after chemotherapy for advanced gastric cancer,maintain the stability of intestinal floras,enhance immunity and improve nutrition status.
