摘要
目的探究丹栀逍遥散加减联合人工泪液(即0.3%玻璃酸钠滴眼液)医治肝经郁热型干眼的临床疗效。方法研究对象为2020年7月—2021年6月于辽宁中医药大学附属医院眼科确诊为肝经郁热型干眼60例患者,随机将其分为两组,中药组和对照组,每组30例。两组患者皆给与人工泪液滴眼治疗14 d,中药组则加用丹栀逍遥散加减方治疗。评估患者于治疗前后临床治疗效果以及诊断干眼的相关指标变化情况。结果通过14 d的治疗,中药组、对照组的总有效率分别为90.00%、66.67%,中药组的总有效率更高(P<0.05);两组眼表疾病指数(ocular surface disease index,OSDI)量表干眼症状评分皆较治疗前有所降低(P<0.05),中药组降低幅度更明显(P<0.05);两组治疗后泪膜破裂时间(breakup time,BUT)、泪液分泌试验(schirmer test,SIT)增加(P<0.05),中药组延长更为显著,角膜荧光染色(corneal fluorescein staining,CFS)则不具有统计学意义(P>0.05)。结论丹栀逍遥散加减联合人工泪液能够在一定程度上缓解肝经郁热型干眼患者的临床症状,明显改善患者泪液分泌量、BUT以及CFS等诊断干眼的理化指标,对于恢复角膜上皮完整作用明显。
Objective To investigate the clinical ffcacy of Moified Danzhi Xiaoyao Powder(丹梔逍遥散加减)combined with artificial tears(i.e.0.3%sodium glass acid eye drops)in the treatment of dry eyes with heat stagnation in liver meridian type.Methods From July 2020 to June 2021,60 patients diagnosed as dry eyes with heat stagnation in liver meridian type were randomly divided into the traditional Chinese medicine group and the control group,with 30 patients in each group.Both groups were treated with artificial tear drops for 14 days,while the traditional Chinese medicine group was treated with Modified Danzhi Xiaoyao Powder.The clinical treatment ffcts and the changes in the relevant indexes of dry eye were assessed before and after the treatment.Results After 14 days of treatment,the total effective rate of the traditional Chinese medicine group and the control group were 90.00%and 66.67%,respectively.The total ffective rate of the traditional Chinese medicine group was higher(P<0.05).The dry eye symptom scores on the ocular surface disease index(OSDI)scale were decreased in both groups(P<0.05),and the decrease in the traditional Chinese medicine group was more significant(P<0.05).The flourescein breakup time(BUT)and Schirmer test(SIT)were increased in both groups after treatment(P<0.05),and the increase was more obvious in the tradi-tional Chinese medicine group,and the cormeal fluorescence staining(CFS)was not statistically significant(P>0.05)。Conclu-sion The combination of Danzhi Xiaoyao Powder and artificial tears can somewhat aleviate the clinical symptoms of dry eyes with heat stagnation in liver meridian,and sigificantly improve the diagnostic indexes of dry eyes such as tear secretion,cormeal fluo-rescein staining and flourescein breakup time,and has a significant effect on the restoration of the integrity of corneal epithelium.
