美乐托宁改善家兔肾盂输尿管连接部梗阻的肾排泄功能障碍及TGF-β1过表达 被引量:1

摘要 目的探讨美乐托宁对家兔肾盂输尿管连接部梗阻(ureteropelvic junction obstruction,UPJO)的肾排泄功能及TGF-β1表达的影响。方法成年新西兰兔随机平均分成假手术组、UPJO模型组、美乐托宁治疗组;仅模型组及治疗组手术制作左侧肾盂输尿管连接部(ureteropelvic junction,UPJ)梗阻模型,治疗组加喂美乐托宁。术前及术后相应时间观察左肾显影时间及肾小球滤过率(glomerular filtration rate,GFR),术后第8周免疫组织化学检测左肾TGF-β1表达,同时检测左UPJ组织学(HE染色)及TGF-β1 mRNA表达。结果与假手术组相比,模型组左肾显影时间延长,GFR降低,且二者随术后时间推移不断加重,同时肾TGF-β1免疫反应性明显增强,UPJ组织病理学变化显著加重,肾及UPJ TGF-β1 mRNA表达明显增加;与模型组相比,美乐托宁治疗组上述变化明显减轻,左肾显影时间缩短、GFR显著增加,但较假手术组除术后第一周相近外,其余各周仍加重,同时肾TGF-β1免疫反应性变化及UPJ组织学病理改变显著好转,肾及UPJ TGF-β1 mRNA表达明显减少,但较假手术组仍加重。结论美乐托宁可改善家兔UPJO和抑制肾TGF-β1表达,缓解纤维化,改善显影时间及GFR,保护肾排泄功能。 Objective To investigate the effect of melatonin on renal excretory function and TGF-β1 expression in rabbits with ureteropelvic junction obstruction(UPJO).Methods Adult New Zealand rabbits were randomly divided into sham operation group,UPJO model group and melatonin treatment group on average.Left UPJO model were established only in UPJO model and melatonin treatment group and melatonin was fed to rabbits only in treatment group.The intravenous pyelogram development time and glomerular filtration rate(GFR)was observed at pre-operation and corresponding postoperative weeks.The immunohistochemistry and mRNA expression of TGF-β1 in left kidney were measured at the postoperative 8 week,along with the measurement of the HE staining and TGF-β1 mRNA expression of left ureteropelvic junction(UPJ).Results Compared with the sham operation group,the development time was significantly prolonged and GFR of left kidney was significantly reduced in the UPJO model group,and both of them were continuously aggravate with the postoperative time lapse.Meanwhile,the kidney TGF-β1 immunoreactivity and UPJ histopathology were significantly increased,and the TGF-β1 mRNA expression of kidney and UPJ was significantly increased.Compared with the UPJO model group,the development time and GFR of left kidney were significantly improved in the treatment group,but they were still more aggravated than the sham operation group except the postoperative first weeks similar.Meanwhile,the changes of TGF-β1 immunoreactivity and UPJ histopathology in the kidney were significantly improved,and the TGF-β1 mRNA expression of kidney and UPJ was significantly decreased,but they were still aggravated in the sham group.Conclusion Melatonin can inhibit TGF-β1 expression of UPJ and kidney in rabbits with UPJO,relieving their fibrosis,improving the development time and glomerular filtration rate,and protect renal excretory function.
