摘要
目的分析沙库巴曲缬沙坦联合酒石酸美托洛尔片治疗高血压合并心力衰竭的临床效果及不良反应。方法选择2021年1月至12月我院收治的80例高血压合并心力衰竭患者作为研究对象,采用数字随机法将其分为观察组(40例,沙库巴曲缬沙坦+酒石酸美托洛尔片)和对照组(40例,酒石酸美托洛尔片)。比较两组的治疗效果、血压、心功能指标、炎性因子水平、不良反应发生情况、N端脑钠肽前体(NT-proBNP)、和肽素(Copeptin)、脂肪细胞因子(Apelin)水平及36项简明健康状况调查表(SF-36)评分。结果观察组的治疗总有效率为95.00%,高于对照组的80.00%(P<0.05)。治疗后,观察组的舒张压、收缩压均低于对照组(P<0.05)。治疗后,观察组的左心室射血分数(LVEF)高于对照组,左心室舒张末期内径(LVEDD)、心率(HR)低于对照组(P<0.05)。治疗后,观察组的肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)水平均低于对照组(P<0.05)。两组的不良反应总发生率比较,差异无统计学意义(P>0.05)。治疗后,观察组的NT-proBNP、Copeptin水平低于对照组,Apelin水平高于对照组(P<0.05)。治疗后,观察组的SF-36各项目评分均高于对照组(P<0.05)。结论沙库巴曲缬沙坦联合酒石酸美托洛尔片治疗高血压合并心力衰竭的临床效果显著,可降低血压及炎性因子水平,改善心功能,且用药安全性较高,值得临床推广和应用。
Objective To analyze the clinical effect and adverse reactions of sacubitril valsartan combined with metoprolol tartrate tablets in the treatment of hypertension and heart failure.Methods A total of 80 patients with hypertension and heart failure admitted in our hospital from January to December 2021 were selected as research objects,and the patients were randomly divided into observation group(40 cases,sacubitril valsartan+metoprolol tartrate tablets)and control group(40 cases,metoprolol tartrate tablets)by digital randomization method.The therapeutic effect,blood pressure,cardiac function indexes,inflammatory factors levels,adverse reaction,N-terminal pro-brain natriuretic peptide(NT-proBNP),Copeptin,adipocyte factor(Apelin)levels and 36-Item Short Form Health Survey Questionnaire(SF-36)scores were compared between the two groups.Results The total effective rate in the observation group was 95.00%,which was higher than 80.00%in the control group(P<0.05).After treatment,the diastolic blood pressure and systolic blood pressure in the observation group were lower than those in the control group(P<0.05).After treatment,the left ventricular ejection fraction(LVEF)in the observation group was higher than that in the control group,while the left ventricular end diastolic diameter(LVEDD)and heart rate(HR)were lower than those in the control group(P<0.05).After treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and hypersensitive C-reactive protein(hs-CRP)in the observation group were lower than those in the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).After treatment,the levels of NT-proBNP and Copeptin in the observation group were lower than those in the control group,and the level of Apelin was higher than that in the control group(P<0.05).After treatment,the scores of cach item of SF-36 in the observation group were higher than those in the control group(P<0.05).Conclusion Sacubitril valsartan combined with metoprolol tartrate tablets in the treatment of hypertension and heart failure has a significant clinical effect,it can reduce blood pressure and inflammatory factors levels,improve cardiac function,and has higher drug safety,which is worthy of clinical promotion and application.
