摘要
目的分析实时三维超声心动图(RT-3DE)与门控心肌灌注显像(GMPI)对评估急性心肌梗死患者经皮冠状动脉介入术(PCI)术后左心功能的应用价值。方法回顾性研究,选取2019年1月至2020年11月运城市中心医院收治的50例急性心肌梗死患者作为研究对象(观察组),分别于术前术后采用RT-3DE与GMPI检测其左心功能指标[包括左室射血分数(LVEF)、左室舒张末期容积(LVEDV)和左室收缩末期容积(LVESV)],另选30名同期健康体检的志愿者作为对照组并测定其左心功能指标参数。对比分析两组测量手段所测参数并采用Pearson分析二者之间的相关性。结果RT-3DE与GMPI所测结果均显示,观察组术前及术后1个月的LVEF均低于对照组,LVEDV和LVESV均高于对照组,差异均有统计意义(P<0.05)。RT-3DE与GMPI所测LVEF、LVEDV和LVESV相比,差异无统计意义(P>0.05)。Pearson分析结果显示,两种方法所测LVEF、LVEDV和LVESV均呈正相关(r术前=0.437、0.917、0.936,r术后=0.495、0.899、0.758,P<0.05)。结论三维超声心动图与门控心肌灌注显像对评估急性心肌梗死患者PCI术后左心功能具有较好的应用价值,临床可酌情联合应用。
Objective To analyze the application value of real-time three-dimensional echocardiography(RT-3DE)and gated myocardial perfusion imaging(GMPI)in evaluating left ventricular function after percutaneous coronary intervention(PCI)in patients with acute myocardial infarction.Methods Fifty patients with acute myocardial infarction admitted to Yuncheng Central Hospital from January 2019 to November 2020 were retrospectively selected as the research subjects(observation group).The left cardiac function indexes(including left ventricular ejection fraction(LVEF),left ventricular end diastolic volume(LVEDV)and left ventricular end systolic volume(LVESV)were measured by RT-3DE and GMPI(observation group),and 30 patients were selected as the study subjects(observation group)volunteers in the health examination were used as control group and their left heart function parameters were measured.The parameters measured by the two sets of measurement methods were compared and analyzed and the correlation between the parameters measured by the two methods was analyzed by Pearson correlation analysis.Results The results of T-3DE and GMPI showed that the LVEF of the observation group before operation and 1 month after operation were lower than those of the control group,and the LVEDV and LVESV of the observation group were higher than those of the control group,and the differences were statistically significant(P<0.05).There was no statistically significant difference between LVEF,LVEDV and LVESV measured by RT-3DE and GMPI(P>0.05).Pearson analysis showed that LVEF,LVEDV and LVESV measured by the two methods were positively correlated(r pre=0.437,0.917,0.936,r post=0.495,0.899,0.758,P<0.05).Conclusion Three-dimensional echocardiography and gated myocardial perfusion imaging have good application value in assessing left ventricular function after PCI in patients with acute myocardial infarction,and can be combined as appropriate in clinical practice.
