^(18)F-FDG PET-CT联合放疗治疗依从性较差食管癌的临床价值分析

摘要 目的探究^(18)F-FDG PET-CT联合放疗治疗依从性较差食管癌的临床价值分析。方法回顾性分析符合标准的96例食管癌患者资料,按照其实际治疗情况分为对照组(常规放疗,n=52)和观察组(^(18)F-FDG PET-CT联合放疗,n=44)。比较两组患者治疗后临床结果、靶区勾画(GTV、CTV)体积、危及器官(OAR)受量和不良反应情况。结果观察组缓解率为77.27%,明显高于对照组的57.70%(P=0.003);观察组总体临床治疗效果明显优于对照组(P=0.001)。观察组勾画的GTV、CTV体积均明显小于对照组(P均<0.001)。观察组勾画的靶区OAR(包括肺、心脏、脊髓和气管)受量均明显低于对照组(P均<0.001)。两组均出现急性放射性肺炎、食管炎、骨髓抑制和消化道的不良反应,其不良反应分级无明显差异(P均>0.05)。结论^(18)F-FDG PET-CT联合放疗治疗依从性较差食管癌有助于精准勾画靶区,消除CT分辨率不足,有效保护正常组织,更好帮助进行放射治疗,提高疾病临床治疗效果。 Objective To investigate the clinical value of ^(18)F-FDG PET-CT combined with radiotherapy for esophageal cancer with poor compliance.Methods Retrospective analysis of 96 cases of esophageal cancer were conducted,and they were divided into the control group(conventional radiotherapy,n=52)and the observation group(^(18)F-FDG PET-CT combined with radiotherapy,n=44).The clinical results,target volume delineation(GTV,CTV)volume,organ at risk(OAR)volume and adverse reactions were compared between the 2 groups after treatment.Results The remission rate of the observation group was 77.27%,which was significantly higher than that of the control group(57.70%)(P=0.003).The overall clinical treatment effect of the observation group was significantly better than that of the control group(P=0.001).The volumes of GTV and CTV delineated in the observation group were significantly smaller than those in the control group(all P<0.001).The OAR doses(including lung,heart,spinal cord and trachea)delineated in the observation group were significantly lower than those in the control group(all P<0.001).Adverse reactions of acute radiation pneumonitis,esophagitis,bone marrow suppression and digestive tract were found in both groups,and there was no significant difference in the grades of adverse reactions(all P>0.05).Conclusion ^(18)F-FDG PET-CT combined with radiotherapy in the treatment of esophageal carcinoma with poor compliance can help to accurately delineate the target area,eliminate the lack of CT resolution,effectively protect normal tissue,better assist in radiotherapy,and improve the clinical treatment effect of the disease.
