代谢综合征与听力损失关联的大样本分析 被引量:1

摘要 目的研究代谢综合征(metabolic syndrome,MS)与听力损失(hearing loss,HL)之间的关联。方法采用横断面研究,研究人群为开滦研究中2014-2019年参加随访且完成纯音测听(pure tone audiometry,PTA)的开滦集团在职及离退休男性员工共23329人。HL按照2021年WHO听力损失的诊断标准进行分级,MS采用2004年美国国家胆固醇教育计划成人治疗小组修订后的诊断标准,并修改了腰围切点。研究人群根据是否符合MS分为两组,并对两组人群的一般情况进行比较,采用Logistic回归模型分析MS与HL之间的关联。结果校正潜在的混杂因素后,MS组听力损失的OR值为1.142(95%CI:1.057-1.234)。与非MS组相比,符合3项及4、5项MS诊断标准人群患HL的OR值为1.117(95%CI:1.022-1.222)、1.191(95%CI:1.063-1.334)。MS组成成分中血糖升高(OR,1.136;95%CI:1.054-1.225)、腹型肥胖(OR,1.124;95%CI:1.045-1.210)、血压升高(OR,1.109;95%CI:1.026-1.199)也与听力损失呈正相关。结论MS与HL呈正向关联,MS可能是HL发生的独立危险因素,MS人群随着代谢异常数目的增加,HL的风险也逐渐升高,早期干预MS的发生发展,特别是血糖升高、腹型肥胖及血压升高是减少HL发病的有效措施。 Objective To determine the association between metabolic syndrome(MS)and hearing loss(HL).Methods This cross-sectional study involved 23,329 active or retired male employees of the Kailuan Group who completed pure tone audiometry(PTA)and underwent follow-up from 2014 to 2019.HL was graded according to WHO diagnosis standards(2021),and MS was determined according to the diagnosis standards amended by the National Cholesterol Education Programme Adult Treatment Panel III(2004),with modified waist circumference cut-off point.Subjects were assigned into two groups according to their MS diagnosis codes.The general characteristics of the two groups were compared and the logistic regression model was used to determine the association between MS and HL.Results After adjustment for potential confounders,the OR value of HL in the MS group was 1.142(95%CI:1.057–1.234).Compared with the non-MS group,the OR values of HL in the group meeting 3,4 and 5 diagnostic criteria were 1.117(95%CI:1.022-1.222)and 1.191(95%CI:1.063-1.334),respectively.The MS components,including hyperglycemia(OR=1.136;95%CI:1.054-1.225),abdominal fat(OR=1.124;95%CI:1.045-1.210)and elevated blood pressure(OR=1.109;95%CI:1.026-1.199),were positively associated with HL.Conclusions MS is positively associated with HL,and MS may be an independent risk factor for HL.As metabolic abnormality numbers increase,the risk of HL gradually increases.Early intervention in the occurrence and development of MS,especially targeting hyperglycemia,abdominal fat and elevated blood pressure,is an effective measure to reduce the incidence of HL.
