摘要
目的 探析缺血性中风(AIS)应用针药联合策略(健脾熄风活络汤+醒脑开窍针刺法)的临床疗效及对中医证候积分、Barthel指数、血流变指标的影响。方法 选择洪洞县中医医院2020年8月—2021年7月中医内科就诊的740例中风病患者,以随机数字表法分为2组各370例。2组均给予标准西医基础及醒脑开窍针刺法治疗,观察组同时联合健脾熄风活络汤治疗。对比2组中风病患者临床疗效、治疗前后2周中医证候积分、Barthel指数(BI)、血液流变学指标[血浆黏度(PV)、高切全血黏度(HBV)及低切全血黏度(LBV)]。结果 2组治疗前中风病中医证候积分、Barthel指数、PV、HBV、LBV差异无统计学意义(P>0.05);治疗后观察组中风病证候积分、PV、HBV、LBV明显低于对照组(P<0.05);而综合疗效、Barthel指数评分均明显高于对照组(P<0.05)。结论 针药联合策略(健脾熄风活络汤+醒脑开窍针刺法)应用于AIS中临床效果显著,可有效改善患者临床症状及血液流变学,提高生理自理能力。
Objective To explore the clinical efficacy of acupuncture and medicine combined strategy( Jianpi Xifeng Huoluo decoction and Xingnao Kaiqiao acupuncture method) for ischemic stroke( AIS) and influence on the scores of traditional Chinese medicine( TCM)syndromes,Barthel index,and hemorheological indexes.Methods A total of 740 stroke patients who were treated in Hongdong Hospital of Traditional Chinese Medicine from August 2020 to July 2021 were selected and divided into two groups,with 370 cases each,by random number table.Both groups were treated with standard western medicine and Xingnao Kaiqiao acupuncture treatment.The observation group was also treated with Jianpi Xifeng Huoluo decoction.The clinical efficacy of the two groups of stroke patients,the scores of TCM syndromes,Barthel index( BI),blood rheology indexes [plasma viscosity( PV),high-cut whole blood viscosity( HBV) ] and low-cut whole blood2 weeks before and after treatment Blood viscosity( LBV) was compared between the two groups.Results Before treatment,there was no significant difference in stroke syndrome scores,Barthel index,PV,HBV,LBV between the two groups( P >0.05).After treatment,stroke syndrome scores,PV,HBV,LBV in the observation group were significantly lower than the control group( P< 0.05),and the improvement rate of syndrome and Barthel index score were significantly higher than those in the control group( P< 0.05).Conclusion The combination of acupuncture and medicine( Jianpi Xifeng Huoluo decoction and Xingnao Kaiqiao acupuncture) has a significant clinical effect in AIS,which can effectively improve patients’ clinical symptoms and blood rheology,and improve their physiological self-care ability.
