摘要
目的:探析在尘肺病检查和诊断中应用直接数字化X射线摄影的临床价值。方法:择取2021年10月~2022年8月本院接受水泥厂粉尘接触史体检的223例作为研究对象,所选研究对象均实施直接数字化X射线摄影技术检查,对胸片质量进行分析,同时对肺部区域小阴影数目以及密集度一致性予以分析,并对职业性尘肺病疾病分期一致性进行对比。结果:①胸片质量:直接数字化X射线摄影技术检查并无废片和三级质量的胸片,213例一级质量胸片,占比95.52%,10例二级质量胸片,占比4.48%。②肺部区域小阴影数目以及密集度:223例受检者中,共涉及到肺区1338个小阴影,直接数字化X射线摄影技术检查发现200个肺区小阴影,检出率14.95%;在肺区小阴影密集度上分析,直接数字化X射线摄影技术检查中0级占比15.70%(35/223),1级占比30.94%(69/223),2级占比32.74%(73/223),3级占比20.63%(46/223)。③职业性尘肺病的疾病分期:直接数字化X射线摄影技术检查的总阳性率10.76%(24/223),其中0期36例,占比16.14%,1期22例,占比9.87%,2期1例,占比0.45%,3期1例,占比0.45%,正常163例,占比73.09%。结论:在尘肺病检查及诊断中,直接数字化X射线摄影图像质量更佳,可以更好地显示出小阴影密集度,方便对尘肺病分期的诊断及明确诊断,使误诊率、漏诊率降低,临床价值显著。
Objective:To explore the value of direct digital radiography in the examination and diagnosis of pneumoconiosis.Methods:A total of 223 people who received physical examination of dust exposure history in cement factory from October 2021 to August 2022 were selected and examined by direct digital radiography.The quality of chest radiography,the number and intensity of small shadows in the lung area,and the consistency of disease stage of occupational pneumoconiosis were analyzed.Results:①Chest film quality:There was no waste or third grade chest film in direct digital X-ray photography.213 cases had first grade chest films,accounting for 95.52%,and 10 cases had second grade chest films,accounting for 4.48%.②Number and density of small shadows in the lung area:1338 small shadows in the lung area were involved in 223 subjects,and 200 small shadows in the lung area were found by direct digital X-ray photography,with a detection rate of 14.95%;According to the analysis of small shadow density in the lung area,grade 0 accounted for 15.70%(35/223),grade 1 accounted for 30.94%(69/223),grade 2 accounted for 32.74%(73/223),and grade 3 accounted for 20.63%(46/223)in direct digital radiography.③Staging of occupational pneumoconiosis:the total positive rate of direct digital radiography was 10.76%(24/223),including 36 cases in stage 0(16.14%),22 cases in stage 1(9.87%),1 case in stage 2(0.45%),1 case in stage 3(0.45%),and 163 cases in normal(73.09%).Conclusion:In the examination and diagnosis of pneumoconiosis,the quality of direct digital X-ray images is better,which can better show the density of small shadows,facilitate the diagnosis and definite diagnosis of pneumoconiosis stage,and reduce the rate of misdiagnosis and missed diagnosis,with significant clinical value.
