摘要
目的探讨还原型谷胱甘肽联合门冬胰岛素治疗妊娠期糖尿病(GDM)的效果及对氧化应激指标、妊娠结局的影响。方法选择2018年8月至2020年10月收治的90例GDM患者为研究对象,按照随机数字表法将其分为对照组与观察组,各45例。对照组接受常规干预+门冬胰岛素,观察组在对照组基础上加用还原型谷胱甘肽。比较两组的治疗效果。结果观察组的治疗总有效率明显高于对照组(P<0.05)。治疗后,观察组的超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)水平高于对照组,丙二醛(MDA)水平低于对照组(P<0.05)。治疗后,观察组的空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)低于对照组,胰岛素β细胞功能指数(HOMA-β)高于对照组(P<0.05)。治疗后,观察组的热休克蛋白60(HSP60)、Nesfatin-1水平低于对照组(P<0.05)。观察组的不良妊娠结局总发生率低于对照组(P<0.05)。结论还原型谷胱甘肽联合门冬胰岛素治疗GDM的效果满意,可有效减轻机体氧化应激损伤,调节HSP60、Nesfatin-1水平,改善胰岛功能及妊娠结局,值得推广和应用。
Objective To explore the effect of reduced glutathione combined with insulin aspart in the treatment of gestational diabetes mellitus(GDM)and its influences on oxidative stress indexes and pregnancy outcome.Methods Ninety GDM patients admitted from August 2018 to October 2020 were selected as the study objects and divided into control group and observation group according to random number table method,with 45 cases in each group.The control group received routine interventien+insulin aspart,and the observation group added reduced glutathione on the basis of the control group.The therapeutic effects of the two groups were compared.Results The total effective rate of treatment in the observation group was significantly higher than that in the control group(P<0.05).After treatment,the levels of superoxide dismutase(SOD)and glutathione peroxidase(GSH-Px)in the observation group were higher than those in the control group,while the level of malondialdehyde(MDA)was lower than that in the control group(P<0.05).After treatment,fasting insulin(FINS)and homeostasis model assessment of insulin resistance index(HOMA-IR)in the observation group were lower than those in the control group,and homeostasis model assessment of isletβ-cell function index(HOMA-β)was higher than that in the control group(P<0.05).After treatment,the levels of heat shock protein 60(HSP60)and Nesfatin-1 in the observation group were lower than those in the control group(P<0.05).The total incidence of adverse pregnancy outcomes in the observation group was lower than that in the control group(P<0.05).Conclusion The reduced glutathione combined with insulin aspart in the treatment of GDM has satisfactory effect,it can effectively reduce oxidative stress injury,regulate the levels of HSP60 and Nesfatin-1,and improve the pancreatic islets function and pregnancy outcome,which is worth promoting and applying.
