摘要
目的探讨人工股骨头置换术(AFHR)对移位型股骨颈骨折(FNF)患者术后功能恢复的影响因素。方法选取2017年1月至2022年5月辽宁省朝阳市第二医院收治的80例FNF患者作为研究对象,所有患者均进行AFHR治疗,按术后恢复效果分为两组,其中预后较好者共67例为N组,预后不良者13例为M组,记录患者的情况并分析其功能恢复的影响因素。结果单因素分析结果显示,患者的年龄(>70岁)、白蛋白水平、病情分级、术后护理效果、不良事件出现情况及骨密度等比较,差异有统计学意义(P<0.05);多因素分析结果显示,患者的白蛋白水平(β=0.661,OR=5.886,95%CI:2.364~11.791)、年龄(>70岁)(β=0.706,OR=18.501,95%CI:4.841~46.452)、病情分级(Garden对线指数Ⅳ级)(β=1.211,OR=3.375,95%CI:1.922~7.245)、术后无康复护理(β=0.006,OR=24.879,95%CI:1.320~58.293)、有不良事件(β=0.112,OR=10.905,95%CI:3.814~21.904)、骨密度疏松(β=0.023,OR=17.469,95%CI:1.394~56.407)是影响FNF患者AFHR术后功能恢复的主要危险因素(P<0.05)。结论采用AFHR治疗移位型FNF在多种因素的影响下容易出现术后并发症,影响康复效果,临床中需针对这些因素进行预防,为患者功能恢复提供借鉴。
Objective To investigate the influence factors of artificial femoral head replacement(AFHR)on postoperative functional recovery of displaced femoral neck fracture(FNF)patients.Methods A total of 80 cases of FNF patients admitted to the Second Hospital of Chaoyang City in Liaoning Province from January 2017 to May 2022 were selected and analyzed.All patients received AFHR treatment,and they were divided into two groups according to the postoperative recovery effect,among them,67 patients with good prognosis were in group N,and 13 patients with poor prognosis were in group M.The patient's condition was recorded and the influencing factors of functional recovery were analyzed.Results The results of univariate analysis showed that there were statistically significant differences in patients'age(>70 years old),albumin level,disease classification,postoperative nursing effect,adverse events and bone mineral density(P<0.05).The results of the multivariate analysis showed that the patient's albumin level(β=0.661,OR=5.886,95%CI:2.364-11.791),age(>70 years old)(β=0.706,OR=18.501,95%CI:4.841-46.452),disease classification(Garden alignment indexⅣ)(β=1.211,OR=3.375,95%CI:1.922-7.245),no rehabilitation care after operation(β=0.006,OR=24.879,95%CI:1.320-58.293),adverse events(β=0.112,OR=10.905,95%CI:3.814-21.904),bone density porosity(β=0.023,OR=17.469,95%CI:1.394-56.407)were the main risk factors affecting the functional recovery of patients with FNF after AFHR(P<0.05).Conclusion The use of AFHR in the treatment of displaced FNF is prone to postoperative complications under the influence of various factors,affecting the rehabilitation effect.These factors should be prevented in clinical practice to provide reference for patients'functional recovery.
