摘要
目的:研究不同电刺激方案联合生物反馈治疗盆底功能障碍性疾病(PFD)效果及对盆底电生理指标影响。方法:选择松滋市妇幼保健院2021年1-12月收治的PFD患者520例,采用随机数字表法将患者分为常规组及增强组,每组260例。常规组给予常规电刺激,而增强组给予增强电刺激,两组均接受生物反馈治疗。治疗8周后评估比较两组治疗效果,比较两组治疗前后盆底肌力及盆底电生理指标,盆底电生理指标包括前静息、慢肌、后静息测试阶段的平均肌电值及快肌最大肌电值。结果:增强组治疗效果优于常规组,差异有统计学意义(P<0.05)。治疗前两组盆底肌力分级比较,差异无统计学意义(P>0.05);治疗后,两组盆底肌力分级均优于治疗前,且增强组优于常规组(P<0.05)。两组治疗前各盆底电生理指标比较,差异均无统计学意义(P>0.05);治疗后,两组前静息平均肌电值、快肌最大肌电值、慢肌平均肌电值、后静息平均肌电值均高于治疗前,且增强组均高于常规组,差异均有统计学意义(P<0.05)。结论:与常规电刺激相比,增强电刺激联合生物反馈治疗PFD效果显著,可明显改善患者盆底肌力及盆底电生理指标。
Objective: To study the effect of different electrical stimulation schemes combined with biofeedback on pelvic floor dysfunction(PFD) and its effect on pelvic floor electrophysiological indexes. Method: A total of 520 patients with PFD treated in Songzi Maternal and Child Health Centre from January to December 2021were selected. The patients were randomly divided into routine group and enhancement group, with 260 cases in each group. The routine group was given routine electrical stimulation, while the enhancement group was given enhanced electrical stimulation. Both groups received biofeedback treatment. After 8 weeks of treatment, the treatment effects of the two groups were evaluated and compared. The pelvic floor muscle strength and pelvic floor electrophysiological indexes of the two groups before and after treatment were compared. The pelvic floor electrophysiological indexes included the average EMG value of pre resting, slow muscle and post resting test stages and the maximum EMG value of fast muscle. Result: The therapeutic effect of the enhancement group was better than that of the routine group(P<0.05). There was no significant difference in pelvic floor muscle strength between the two groups before treatment(P>0.05);after treatment, the pelvic floor muscle strength grades of the two groups were better than those before treatment, and the enhancement group was better than the routine group(P<0.05). There were no significant differences in pelvic floor electrophysiological indexes between the two groups before treatment(P>0.05);after treatment, the pre resting mean EMG value, the maximum EMG value of fast muscle, the mean EMG value of slow muscle and the post resting mean EMG value of the two groups were higher than those before treatment, and those in the enhancement group were better than those in the routine group, the differences were statistically significant(P<0.05). Conclusion: Compared with conventional electrical stimulation, enhanced electrical stimulation combined with biofeedback is effective in the treatment of PFD, which can significantly improve the pelvic floor muscle strength and pelvic floor electrophysiological indexes.
