SLE患者INF-α血清水平变化预测模型的建立和意义

摘要 目的干扰素α(IFN-α)参与了系统性红斑狼疮(systemic lupus eryematosus,SLE)的发病,但在不同临床表型中的作用不同,本研究拟根据SLE患者不同临床表型建立IFN-α血清水平的预测模型,为筛选靶向干扰素治疗患者提供参考。方法检测SLE患者和健康对照组IFN-α的血清水平,以健康对照组IFN-α“均值±2×标准差”为界,高于“均值+2×标准差”定义为升高组,低于“均值-2×标准差”为降低组;根据患者不同临床特征,采用多项logistic回归模型分析,建立预测干扰素血清水平变化的预测模型。结果筛选了96例初发或复发SLE患者和29例健康对照者,健康对照组IFN-α的血清水平为100.4122.37(pg/mL);多项Logistic回归分析显示,IFN-α降低的预测模型G1=LOG[P(降低)/P(正常)]=-36.90-15.29无皮疹+0.83无肾炎+1.74无关节炎+16.73无血小板减少+16.28无其他脏器损害;IFN-α升高的预测模型G2=LOG[P(升高)/P(正常)]=-1.55+1.53无皮疹+0.03无肾炎-0.01无关节炎+0.95无血小板减少-0.07无其他脏器损害,其中有无皮疹预测价值有统计学意义(P=0.023);预测IFN-α降低的正确百分比为0,预测IFN-α升高的正确百分比为58.7%,预测IFN-α不变的正确百分比为73.3%。结论根据患者有无肾炎、关节炎、皮疹及血小板减少等因素可以预测SLE患者血清IFN-α变化,特别是对预测是否升高具有一定价值。 Objective Interferon-α(IFN-α)Involved in pathogenesis of systemic lupus erythematosus(SLE),but it has different roles in different clinical phenotypes.The study intends to establish IFN-αserum level prediction model according to different clinical phenotypes of SLE patients,and can provide reference for screening patients receiving targeted interferon therapy.Methods Detection of IFN in SLE patients and healthy controls-αSerum level of the healthy control group IFN-α"Mean±2×Standard deviation"as the boundary,higher than"mean+2×The"standard deviation"is defined as the increase group,which is lower than the"mean value-2×Standard deviation"is the decreasing group;According to the different clinical characteristics of patients,multiple logistic regression model was used to establish a predictive model to predict the changes of interferon serum levels.Results The paper chose 96 patients with primary or recurrent SLE and 29 healthy controls,IFN-αserum level was 100.4122.37(pg/mL)in healthy control group;multiple Logistic regression analysis showed that,IFN-αreduced prediction model G1=LOG[P(reduced)/P(normal)]=-36.90-15.29,no rash+0.83,no nephritis+1.74,no arthritis+16.73,no thrombocytopenia+16.28,no other organ damages;IFN-αincrease prediction model G2=LOG[P(increase)/P(normal)]=-1.55+1.53,no rash+0.03,no nephritis-0.01,no arthritis+0.95,no thrombocytopenia-0.07,no other organ damages,in which predictive value of rash was statistically significant(P=0.023).Correct percentage of predicted interferonαdecrease was 0,and correct percentage of predicted interferonαincrease was 58.7%,and correct percentage of predicted interferonαunchanged was 73.3%.Conclusion Serum IFN-αchanges of SLE patients can be predicted according to presence or absence of nephritis,arthritis,rash and thrombocytopenia,which has some certain value especially for predicting its increase.
