摘要
目的分析血常规检测用于鉴别诊断地中海贫血以及缺铁性贫血的价值。方法回顾性分析2021年1月至2021年12月南阳市中心医院收治的疑似地中海贫血患者共80例设为A组,纳入同时段本院收治的疑似缺铁性贫血患者共80例设为B组,另纳入同时间段本院100例健康体检者作为C组,三组均接受血常规检测,将基因检测、骨髓铁染色结果当作诊断地中海贫血以及缺铁性贫血的金标准,分析联合各项血常规检测指标对地中海贫血以及缺铁性贫血的诊断效能,统计A组中α-地中海贫血、β-地中海贫血患者的突变基因型,并比较A、B组确诊地中海贫血和缺铁性贫血患者与C组血红蛋白(HB)、红细胞(RBC)、红细胞和血红蛋白比值(RBC/HB)、平均红细胞比容(MCV)、平均红细胞血红蛋白含量(MCH)、平均红细胞血红蛋白浓度(MCHC)及红细胞体积分布宽度(RDW)水平。结果A组基因检测确诊地中海贫血阳性43例,阴性37例;联合血常规指标显示地中海贫血阳性42例,阴性38例。联合血常规指标对地中海贫血的诊断准确度、敏感度、特异度分别为96.25%、95.35%、97.30%。B组骨髓铁染色缺铁性贫血阳性37例,阴性43例;联合血常规指标显示缺铁性贫血阳性36例,阴性44例。联合血常规指标对缺铁性贫血的诊断准确度、敏感度、特异度分别为93.75%、91.89%、95.35%。A组确诊患者HB、MCV、MCH、MCHC水平低于C组,RBC/HB水平高于C组,差异有统计学意义(P<0.05);B组确诊患者HB、MCV、MCH、MCHC水平低于C组,RBC、RBC/HB、RDW水平高于C组,差异有统计学意义(P<0.05);A组确诊患者HB、RBC水平高于B组确诊患者,RDW水平低于B组确诊患者,差异有统计学意义(P<0.05)。结论血常规检测用于鉴别诊断地中海贫血以及缺铁性贫血的价值较高,其中HB、RBC以及RDW三项指标能作为两类疾病鉴别诊断的重要参考指标。
Objective To analyze the value of routine blood examination in differential diagnosis of thalassemia and iron deficiency anemia.Methods A total of 80 patients with suspected thalassemia admitted to Nanyang Central Hospital from January 2021 to December 2021 were retrospectively analyzed as group A,80 patients with suspected iron deficiency anemia admitted to our hospital at the same time were set as group B,and 100 health examinees in our hospital at the same time were included as group C.All three groups received routine blood tests.The results of gene testing and bone marrow iron staining were taken as the gold standard for diagnosing thalassemia and iron deficiency anemia.The diagnostic efficacy of thalassemia and iron deficiency anemia in combination with various routine blood tests was analyzed,the mutation genotypes ofα-thalassemia andβ-thalassemia patients in group A were count,the levels of hemoglobin(HB),red blood cell(RBC),RBC-to-HB ratio(RBC/HB),mean corpuscular volume(MCV),mean corpuscular hemoglobin(MCH),mean hemoglobin concentration(MCHC)and red blood cell volume distribution width(RDW)were compared between groups A and B and C.Results Group A gene test confirmed that 43 cases were positive and 37 cases were negative for thalassemia;The combined blood routine test showed that 42 cases were positive and 38 cases were negative for thalassemia.The diagnostic accuracy,sensitivity and specificity of the combined blood routine indicators for thalassemia were 96.25%,95.35%and 97.30%respectively.In group B,there were 37 positive and 43 negative cases of iron deficiency anemia with bone marrow iron staining;Combined blood routine test showed that 36 cases were positive and 44 cases were negative for iron deficiency anemia.The diagnostic accuracy,sensitivity and specificity of the combined blood routine indicators for iron deficiency anemia were 93.75%,91.89%and 95.35%respectively.The levels of HB,MCV,MCH and MCHC in group A were lower than those in group C,and the levels of RBC/HB were higher than those in group C,with statistical significance(P<0.05);The levels of HB,MCV,MCH and MCHC in group B were lower than those in group C,while the levels of RBC,RBC/HB and RDW in group B were higher than those in group C(P<0.05);The level of HB and RBC in group A was higher than that in group B,and the level of RDW was lower than that in group B(P<0.05).Conclusion Routine blood examination is of high value in differential diagnosis of thalassemia and iron deficiency anemia,among which HB,RBC and RDW can be used as important reference indicators for differential diagnosis of two kinds of diseases.
