早期肠内营养支持对呼吸机辅助通气的ICU危重症患者疾病转归的影响

摘要 目的:分析早期肠内营养支持对呼吸机辅助通气的重症医学科(ICU)危重症患者疾病转归的影响效果。方法:选取本院ICU 2019年1月至2022年6月收入的80例行呼吸机辅助通气的危重症患者,依据肠内营养支持实施时机分为两组各40例。对照组在入ICU后监测生化指标,存在营养不良下启动肠内营养支持,观察组在收入ICU后24h内开始早期肠内营养支持。分别在入ICU时、转出ICU时采血测定两组患者的营养状态指标、免疫功能,转出ICU时统计营养不良总发生率、肺部感染发生率、平均脱机时间和平均ICU住院时间,对比以上观察指标差异。结果:观察组患者的营养不良总发生率、肺部感染发生率低于对照组(P<0.05),转出ICU时的白蛋白、总蛋白、球蛋白高于对照组(P<0.05),降钙素原低于对照组(P<0.05),平均脱机时间和平均ICU住院时间短于对照组(P<0.05)。结论:早期肠内营养支持可以改善呼吸机辅助通气的ICU危重症患者营养状态、增强其免疫功能,降低营养不良总发生率及肺部感染发生率,缩短脱机及ICU住院时间。 Objective:To analyze the effect of early enteral nutrition support on the prognosis of critically ill patients in ICU with ventilator assisted ventilation.Methods:80 critically ill patients receiving ventilator assisted ventilation from January 2019 to June 2022 in ICU of our hospital were selected and divided into two groups according to the implementation time of enteral nutrition support,40 patients in each group.The control group monitored the biochemical indicators after entering the ICU,and started enteral nutrition support in case of malnutrition.The observation group started early enteral nutrition support within 24 hours after entering the ICU.Blood samples were taken at the time of admission to the ICU and transfer out of the ICU to determine the nutritional status indicators and immune function of the two groups of patients.The total incidence of malnutrition,the incidence of pulmonary infection,the average offline time and the average length of stay in the ICU were counted at the time of transfer out of the ICU,and the differences of the above observation indicators were compared.Results:The total incidence of malnutrition and pulmonary infection in the observation group was lower than that in the control group(P<0.05).Conclusion:Early enteral nutrition support can improve the nutritional status of critically ill ICU patients with ventilator-assisted ventilation,enhance their immune function,reduce the overall incidence of malnutrition and pulmonary infection,and shorten the offline and ICU hospital stay.
