摘要
目的 探讨不同持续时间的穴位敷贴对气虚血瘀型糖尿病周围神经病变(DPN)患者的影响。方法选择2020年11月至2022年4月于福建中医药大学附属人民医院就诊的126例气虚血瘀型DPN患者,采用随机数表法分为三组,每组各42例。干预(1)组在常规治疗和护理的基础上给予当归补血方于三阴交、涌泉穴位敷贴2 h;干预(2)组在常规治疗和护理的基础上给予当归补血方于三阴交、涌泉穴位敷贴4 h;干预(3)组在常规治疗和护理的基础上给予当归补血方于三阴交、涌泉穴位敷贴6 h;干预时间均为4周,评价患者的血液流变学指标、多伦多临床神经病变评分(TCSS)及不良反应发生情况。结果 三组患者干预后的血液流变学较干预前有所改善,干预后组间全血黏度(1s-1、50s-1、100s-1)、血浆黏度及红细胞聚集指数比较,差异有统计学意义(P <0.05),干预(2)组和干预(3)组优于干预(1)组。三组患者干预后的TCSS较干预前有所改善,干预后三组神经症状、感觉功能的比较,差异有统计学意义(P <0.05),干预(2)组优于干预(1)组和干预(3)组。三组患者不良反应发生情况比较差异有统计学意义(P <0.05),干预(3)组高于干预(1)组和干预(2)组。结论 推荐当归补血方穴位敷贴的最佳时间是4 h,不仅在改善气虚血瘀型DPN患者的肢体症状和血液流变学等方面疗效明显,且安全性较高。
Objective To investigate the impacts of acupoint application with different durations on diabetic peripheral neuropathy(DPN) patients with qi deficiency and blood stasis. Methods A total of 126 DPN patients with qi deficiency and blood stasis who were treated in the People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine from November 2020 to April 2022 were selected as the study subjects, and they were divided into three groups according to the random number table method, with 42 cases in each group. In the intervention group(1), patients were treated with application of Danggui Buxue Recipe on the Sanyinjiao and Yongquan acupoints for 2 hours on the basis of conventional treatment and nursing;In the intervention group(2), patients were treated with application of Danggui Buxue Recipe on the Sanyinjiao and Yongquan acupoints for 4 hours on the basis of conventional treatment and nursing;In the intervention group(3), patients were treated with application of Danggui Buxue Recipe on the Sanyinjiao and Yongquan acupoints for 6 hours on the basis of conventional treatment and nursing. The intervention time was 4 weeks, and the hemorheology indices, Toronto clinical scoring system(TCSS) and the occurrence of adverse reactions(ADRs) were evaluated. Results After intervention, the hemorheology of patients in the three groups was improved compared with that before intervention. After intervention, there were statistically significant differences in the whole blood viscosity(1s-1, 50s-1, 100s-1), plasma viscosity and erythrocyte aggregation index between groups(P < 0.05), with the hemorheology of the intervention group(2) and the intervention group(3) superior to that of the intervention group(1). The TCSS of the three groups of patients after intervention was improved compared with that before intervention. The comparison between the groups after intervention showed that there were statistically significant differences in neurological symptoms and sensory functions(P < 0.05), with the TCSS of the intervention group(2) superior to that of the intervention group(1) and the intervention group(3). There was statistically significant difference in the incidence of ADRs in the three groups(P < 0.05), and the incidence of ADRs of the intervention group(3) was higher than that of the intervention group(1) and the intervention group(2). Conclusion It is recommended that the best duration of acupoint application of Danggui Buxue Recipe is 4 hours, which is not only effective in improving limb symptoms and hemorheology of DPN patients with qi deficiency and blood stasis, but also is relatively safer.
