摘要
目的系统评价针灸治疗带状疱疹后遗神经痛(PHN)的临床疗效。方法检索中国知网(CNKI)、万方数据知识服务平台(万方数据)、维普中文科技期刊(VIP)、中国生物医学文献数据库(CBM)、PubMed、Embase、Web of Science、Cochrane Library建库至2022年4月关于针灸治疗PHN的随机对照试验(RCT)研究文献,使用RevMan 5.4软件进行meta分析。结果最终纳入RCT 18篇,涉及患者1291例。meta分析结果显示,试验组的视觉模拟疼痛量表(VAS)评分[MD=1.55,95%CI(1.11,1.99),Z=6.90,P<0.00001]、总有效率[OR=4.56,95%CI(2.96,7.04),Z=6.86,P<0.00001]、P物质[SMD=2.08,95%CI(1.21,2.94),Z=4.71,P<0.00001]均显著优于对照组。结论针灸疗法可显著缓解PHN患者疼痛症状,但由于纳入文献质量水平参差不齐,存在偏倚因素,因此后续研究者在临床应用证据时需谨慎使用,未来仍需多中心、大样本研究开展进行验证。
Objective To systematically assess the clinical efficacy of acupuncture for postherpetic neuralgia(PHN).Methods Randomized controlled trials(RCT)on the treatment of PHN with acupuncture included in Chinese National Knowledge Internet(CNKI),Wanfang Data Knowledge Service Platform(Wanfang Data),VIP Chinese Journal of Science and Technology(VIP),Chinese Biomedical Literature Database(CBM),PubMed,Embase,Web of Science,and Cochrane Library from the establishment of databases to April 2022 were retrieved,and meta-analysis was performed using RevMan 5.4 software.Results A total of 18 RCTs were included in this study,involving 1291 patients.The results of the meta-analysis showed that the visual analogue scale(VAS)results(MD=1.55,95%CI[1.11,1.99],Z=6.90,P<0.00001),the overall effective rate(OR=4.56,95%CI[2.96,7.04],Z=6.86,P<0.00001),and substance P results(SMD=2.08,95%CI[1.21,2.94],Z=4.71,P<0.00001)in the experimental group were significantly better than those in the control group.Conclusion Acupuncture therapy can significantly alleviate pain symptoms in patients with PHN.And there are still bias factors due to the varying quality levels of the included literature,so subsequent researchers should apply the evidence clinically with caution,and future multicenter and large-size sample studies are still required for validation.
