基于问题式学习健康教育对乳腺癌患者术后心理状态、病耻感及生活质量的影响 被引量:11

Effect of problem-based learning health education on psychological status,stigma and quality of life of breast cancer patients after surgery

摘要 目的探讨基于问题式学习(PBL)健康教育对乳腺癌患者术后心理状态、病耻感及生活质量的影响。方法根据干预方式的不同将124例乳腺癌患者分为观察组(n=63)和对照组(n=61),对照组患者给予常规健康教育,观察组患者给予PBL健康教育。比较两组患者的心理状态[医院焦虑抑郁量表(HADS)]、病耻感(病耻感感受问卷)、生活质量[乳腺癌患者生活质量测定量表(FACT-B)]、护理满意度。结果干预后,两组患者HADS量表焦虑和抑郁评分均低于本组干预前,且观察组HADS量表焦虑和抑郁评分均低于对照组,差异均有统计学意义(P﹤0.05)。干预后,两组患者病耻感感受问卷各维度评分均低于本组干预前,且观察组患者病耻感感受问卷各维度评分均低于对照组,差异均有统计学意义(P﹤0.05)。干预后,两组患者FACT-B量表评分均高于本组干预前,且观察组患者FACT-B量表评分高于对照组,差异均有统计学意义(P﹤0.05)。观察组患者的总满意度为98.41%,明显高于对照组患者的80.33%,差异有统计学意义(P﹤0.01)。两组患者并发症总发生率比较,差异无统计学意义(P﹥0.05)。结论PBL健康教育可明显改善乳腺癌患者术后的焦虑抑郁情绪,减轻病耻感,提高生活质量和护理满意度。 Objective To explore the effect of problem-based learning(PBL)health education on the psychological state,stigma and quality of life of breast cancer patients after operation.Method A total of 124 breast cancer patients were divided into observation group(n=63,received PBL health education)and control group(n=61,received routine health education)according to different intervention methods.The psychological status[hospital anxiety and depression scale(HADS)],stigma(stigma perception questionnaire),quality of life[functional assessment of cancer therapy-breast(FACT-B)]and nursing satisfaction were compared between the two groups.Result After the intervention,the anxiety and depression scores of the HADS scale in the two groups were lower than those before the intervention,and those scores in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After the intervention,the scores of each dimension of the stigma perception questionnaire in the two groups were lower than those before the intervention,and the above scores in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).After intervention,the FACT-B scale scores in both groups were higher than those before the intervention,and the FACT-B scale score in the observation group was higher than that in the control group,the differences were statistically significant(P<0.05).The total satisfaction rate of the patients in the observation group was 98.41%,which was significantly higher than 80.33%in the control group,the difference was statistically significant(P<0.01).There was no significant difference in the total incidence of complication between the two groups(P>0.05).Conclusion PBL health education can significantly improve the anxiety and depression of breast cancer patients after surgery,reduce stigma,and improve the quality of life and nursing satisfaction.
