摘要
目的:分析老年钙化性心脏瓣膜病发生的影响因素。方法:回顾性分析2020年4月至2022年3月该院收治的81例老年钙化性心脏瓣膜病患者的临床资料,设为钙化组,另回顾性分析同期该院收治的80例非钙化性老年心脏瓣膜病患者的临床资料,设为非钙化组,统计两组一般资料,分析老年钙化性心脏瓣膜病的影响因素。结果:两组性别、合并高血压、合并糖尿病、骨钙素>46 ng/mL、降钙素<17.5 pmol/L、钙≤2.25 mmol/L、磷>1.45 mmol/L、镁<1.15 mmol/L、超敏C反应蛋白<10 mg/L、三酰甘油≥1.7 mmol/L占比比较,差异均无统计学意义(P>0.05);两组年龄>65岁、体质量指数≤23 kg/m^(2)、吸烟史、合并冠心病、合并慢性阻塞性肺疾病、甲状旁腺激素(PTH)>87.1 pg/mL、25羟基维生素D<20 ng/mL、血肌酐>120μmol/L、尿素氮>7.14 mmol/L、尿酸>420μmol/L、同型半胱氨酸(Hcy)>15μmol/L、胱抑素C≤1.09 mg/L、总胆固醇≤5.2 mmol/L、低密度脂蛋白胆固醇>3.6 mmol/L占比比较,差异均有统计学意义(P<0.05);Logistic回归分析结果显示,年龄>65岁、PTH>87.1 pg/mL、Hcy>15μmol/L均为老年钙化性心脏瓣膜病发生的危险因素(OR>1,P<0.05)。结论:年龄>65岁、PTH>87.1 pg/mL、Hcy>15μmol/L均为老年钙化性心脏瓣膜病发生的危险因素。
Objective:To analyze influencing factors of senile calcified valvular disease.Methods:The clinical data of 81 elderly patients with calcified valvular heart disease admitted to this hospital from April 2020 to March 2022 were retrospectively analyzed,and these patients were set as calcified group.The clinical data of 80 elderly patients without calcified valvular heart disease admitted to this hospital during the same period were retrospectively analyzed,and they were set as non-calcified group.The general data of the two groups were statistically analyzed.The influencing factors of senile calcified valvular heart disease were analyzed.Results:Two groups of gender,combined hypertension,combined diabetes,osteocalcin>46 ng/mL,calcitonin<17.5 pmol/L,calcium≤2.25 mmol/L,phosphorus>1.45 mmol/L,magnesium<1.15 mmol/L,hypersensitive C-reactive protein<10 mg/L,triacylglycerol≥1.7 There was no significant difference in the proportion of mmol/L(P>0.05).Age>65 years old,body mass index≤23 kg/m^(2),smoking history,coronary heart disease,chronic obstructive pulmonary disease,parathyroid hormone(PTH)>87.1 pg/mL,25 hydroxyvitamin D<20 ng/mL,serum creatinine>120μmol/L,urea nitrogen>7.14 There were statistically significant differences in the proportions of mmol/L,uric acid>420μmol/L,homocysteine(Hcy)>15μmol/L,cystatin C≤1.09 mg/L,total cholesterol≤5.2 mmol/L and low density lipoprotein cholesterol>3.6 mmol/L(P<0.05).Logistic regression analysis showed that age>65 years,PTH>87.1 pg/mL,Hcy>15μmol/L were the risk factors of senile calcified valvular disease(OR>1,P<0.05).Conclusions:Age>65 years,PTH>87.1 pg/mL,Hcy>15μmol/L are the risk factors of senile calcified valvular disease.
