摘要
目的研究降钙素原、D-二聚体、全程C反应蛋白联合检测在脓毒症患儿中的诊断价值。方法随机选取2017年1月至2019年12月在本院就诊的60例疑似脓毒症患儿作为研究对象,均接受降钙素原、D-二聚体、全程C反应蛋白检测。观察所有疑似脓毒症患儿的诊断结果,将确诊的脓毒症患儿分为脓毒症组和严重脓毒症组,比较两组患儿治疗前、后的降钙素原、D-二聚体、全程C反应蛋白水平;比较降钙素原、D-二聚体、全程C反应蛋白单独与联合检测对脓毒症的诊断效能。结果60例患儿中确诊脓毒症36例,严重脓毒症18例;治疗后,两组的降钙素原、D-二聚体、全程C反应蛋白水平低于治疗前,差异具有统计学意义(P<0.05);治疗后,严重脓毒症组的降钙素原、D-二聚体、全程C反应蛋白水平高于脓毒症组,差异具有统计学意义(P<0.05)。以临床最终确诊结果为金标准,联合检测诊断脓毒症的灵敏度、准确度高于降钙素原、D-二聚体、全程C反应蛋白单独检测,特异度高于D-二聚体单独检测,误诊率低于D-二聚体单独检测,漏诊率低于降钙素原、D-二聚体、全程C反应蛋白单独检测(P<0.05)。结论降钙素原、D-二聚体、全程C反应蛋白联合检测能够早期诊断儿童脓毒症并判断病情严重程度。
Objective To study the diagnostic value of combined detection of procalcitonin,D-dimer and full course C-reactive protein in children with sepsis.Methods Sixty children with suspected sepsis treated in our hospital from January 2017 to December 2019 were randomly selected as the study objects,and all of the children received procalcitonin,D-dimer and full course C-reactive protein detection.The diagnostic results of suspected sepsis in all children were observed,children diagnosed with sepsis were divided into sepsis group and severe sepsis group,and the levels of procalcitonin,D-dimer and full course C-reactive protein before and after treatment were compared between the two groups;the diagnostic efficacy of procalcitonin,D-dimer and full course C-reactive protein alone and in combination for sepsis were compared.Results Among the 60 children,36 cases were diagnosed with sepsis and 18 cases with severe sepsis;after treatment,the levels of procalcitonin,D-dimer and full course C-reactive protein in the two groups were lower than those before treatment,and the differences were statistically significant(P<0.05);after treatment,the levels of procalcitonin,D-dimer and full course C-reactive protein in the severe sepsis group were higher than those in the sepsis group,and the differences were statistically significant(P<0.05).Taking final clinical diagnosis as the gold standard,the sensitivity and accuracy of combined detection in the diagnosis of sepsis were higher than those of procalcitonin,D-dimer and full course C-reactive protein alone,the specificity was higher than that of D-dimer alone,the misdiagnosis rate was lower than that of D-dimer alone,and the missed diagnosis rate was lower than that of procalcitonin,D-dimer and full course C-reactive protein alone(P<0.05).Conclusion Combined detection of procalcitonin,D-dimer and full course C-reactive protein can early diagnose sepsis in children and judge the severity of the disease.
