摘要
目的探讨血清总胆红素(TBIL)与尿酸(UA)检验在冠心病患者中的评估价值。方法从2019年1月—2021年9月盐城市大丰中医院收治的冠心病患者中抽取80例为研究组,抽取同期80名健康体检志愿者为对照组。采集两组清晨空腹静脉血样,以西门子ADVIA 1800全自动生化分析仪测定血清TBIL与UA,比较两组差异,评价两项指标单一及联合诊断冠心病的效能。另外,采用冠脉Gensini评分法评价研究组患者冠心病严重程度,观察不同病情严重程度患者血清TBIL与UA差异,分析TBIL与UA与冠心病Gensini评分的相关性。结果研究组血清TBIL(13.67±3.57)μmol/L低于对照组,UA(340.07±68.35)μmol/L高于对照组,差异有统计学意义(t=5.940、2.076,P<0.05)。血清TBIL与UA联合诊断冠心病的灵敏度为81.25%,高于TBIL与UA单一诊断;特异度为85.00%,低于TBIL与UA单一诊断。研究组不同Gensini评分患者血清TBIL与UA比较,差异有统计学意义(P<0.05)。Spearman相关分析示,TBIL与冠脉Gensini评分呈负相关(r=-0.732,P=0.08),UA与冠脉Gensini评分呈正相关(r=0.775,P=0.006)。结论血清TBIL与UA可以为冠心病临床诊断提供有价值的实验室依据,联合应用可以进一步提高诊断灵敏度,亦可为临床评估冠心病严重程度提供参考。
Objective To investigate the evaluation value of serum total bilirubin(TBIL)and uric acid(UA)tests in patients with coronary heart disease.Methods From January 2019 to September 2021,80 patients with coronary heart disease admitted to Yancheng Dafeng Hospital of Traditional Chinese Medicine were selected as the study group,and 80 healthy physical examination volunteers were selected as the control group during the same period.Early morning fasting venous blood samples were collected from both groups,and serum TBIL and UA were measured by Siemens ADVIA 1800 automatic biochemical analyzer to compare the differences between the two groups and evaluate the efficacy of the two indices for single and combined diagnosis of coronary artery disease.In addition,the coronary Gensini score was used to evaluate the severity of coronary heart disease in the study group,to observe the difference between serum TBIL and UA in patients with different disease severity,and to analyze the correlation between TBIL and UA and the Gensini score of coronary heart disease.Results Serum TBIL(13.67±3.57)μmol/L in the study group was lower than that in the control group,and UA(340.07±68.35)μmol/L was higher than that in the control group,and the differences were statistically significant(t=5.940,2.076,P<0.05).The sensitivity of combined serum TBIL and UA for the diagnosis of coronary artery disease was 81.25%,which was higher than that of TBIL and UA alone;the specificity was 85.00%,which was lower than that of TBIL and UA alone.Comparison of serum TBIL and UA in patients with different Gensini scores in the study group,and the differences were statistically significant(P<0.05).Spearman correlation analysis showed that TBIL was negatively correlated with coronary Gensini score(r=-0.732,P=0.008)and UA was positively correlated with coronary Gensini score(r=0.775,P=0.006).Conclusion Serum TBIL and UA can provide valuable laboratory basis for the clinical diagnosis of coronary heart disease,and their combined application can further improve the diagnostic sensitivity and also provide reference for clinical assessment of the severity of coronary heart disease.
