摘要
目的 评价经胸实时三维超声心动图(RT-3DE)TrueVue Light模式在继发孔型房间隔缺损诊断中的临床应用价值。资料与方法 连续性纳入空军军医大学第一附属医院2020年9月1—30日继发孔型房间隔缺损(ASD)患者20例,应用经胸RT-3DE TrueVue Light模式观察其缺损口形态、空间位置及毗邻关系,并与术中实测(缺损口上下径D1及前后径D2)进行比较。用Pearson法分析RT-3DE TrueVue Light测值与术中实测值的相关性。结果 20例ASD患者(共21处缺损,1例双孔)均取得满意的RT-3DE图像。RT-3DE TrueVue Light模式右心房面观察ASD形态以多边形为主(12例),其中9例多边形ASD缺损口纵横比(D1/D2)<1;椭圆形9例,其中7例椭圆形缺损口纵横比>1。二维超声、RT-3DE TrueVue Light和术中实测值D1、D2比较,差异均有统计学意义(χ^(2)=25.00、14.80,P均<0.001);二维超声与RT-3DE TrueVue Light测值D1、D2比较,差异均有统计学意义(χ^(2)=3.887、3.345,P<0.05);二维超声与术中实测值D1、D2比较,差异均有统计学意义(χ^(2)=3.705、3.245,P<0.05);RT-3DE TrueVue Light测值与术中实测值D1、D2的差异均无统计学意义(χ^(2)=1.481、1.446,P>0.05)。Pearson相关分析显示,RT-3DE TrueVue Light模式测值与术中实测值呈线性正相关(r=0.975,P<0.001)。结论 RT-3DE TrueVue Light模式能清晰立体显示ASD缺损口的形态及毗邻结构的前后层次关系,准确测量缺损口大小。
Purpose To evaluate the clinical value of TrueVue Light mode of real-time three-dimensional echocardiography(RT-3DE in the diagnosis of the secundum atrial septal defect(ASD). Materials and Methods Twenty patients with secundum ASD from September 1 to 30, 2020 in the First Affiliated Hospital of the Air Force Military Medical University were consecutively included. The types, spatial location, and adjacent relationship of ASD were observed and compared with the intraoperative measurements(D1 and D2). Pearson method was used to analyze the correlation between RT-3DE TrueVue Light measurement and intraoperative measurement. Results Twenty patients with ASD(total 21 defects on account of 1 double hole) obtained satisfactory RT-3DE images. Observed in TrueVue Light mode, 12 ASD was polygonal and among which the aspect ratio of 9 cases were less than 1, 9 ASD was elliptical and among which the aspect ratio of 7 cases were more than 1. There was significant difference between two-dimensional ultrasound, RT-3DE TrueVue Light and intraoperative measured values D1 and D2(χ^(2)=25.00, 14.80, P<0.001). There was significant difference between two-dimensional ultrasound and RT-3DE TrueVue Light D1 and D2(χ^(2)=3.887, 3.345, P<0.05);There was significant difference between two-dimensional ultrasound and intraoperative measured values D1 and D2(χ^(2)=3.705, 3.245, P<0.05);There was no significant difference between RT-3DE TrueVue Light and intraoperative measured values D1 and D2(χ^(2)=1.481, 1.446, P>0.05). Pearson correlation analysis showed that there was a linear positive correlation between RT-3DE TrueVue Light mode and intraoperative measurement value(r=0.975, P<0.001). Conclusion RT-3DE TrueVue Light mode can clearly and stereoscopically display the shape of ASD defect and the relationship between adjacent structures, accurately measure the size of ASD defect,.
