基于数据挖掘对中医药治疗年龄相关性黄斑变性用药规律研究 被引量:6

摘要 目的探究中医药治疗年龄相关性黄斑变性(AMD)的用药规律。方法以中国知网、万方、维普、中国生物医学文献服务系统、PubMed、Embase、MEDLINE、Web of Science数据库作为查找源,日期设定为2000年1月—2022年9月进行检索,根据纳入标准,将纳入的文献和提取的数据录入到Excel建立数据库,使用SPSS Statistics 25.0及IBM SPSS Modeler 18.0统计软件进行分析,将纳入方剂中的中药进行统计分析。结果(1)高频中药及功效分类:共纳入147篇文献,提取方剂152首,其中出现频数最高的前3位中药是枸杞子(65次,42.76%)、茯苓(63次,41.45%)、当归(61次,40.13%);根据功效进行分类,共涉及7类中药,前3类分别为补虚药(505次,50.91%)、利水渗湿药(117次,11.79%)、活血化瘀药(116次,11.69%);(2)高频中药药性及归经:在药性中,寒(11次,39.28%),温(9次,32.14%),平(8次,28.58%)占比高,无热性药物;在药味统计中,甘(18次,42.9%),苦(10次,23.7%),辛(8次,19.0%)占比高,无咸味药物;在归经统计中,肝经(18次,24.7%),肺经和肾经(各12次,16.4%),脾经(11次,15.1%)占比高;(3)聚类分析:高频药物分为5类,第1类为茯苓、白术、陈皮、当归、白芍、柴胡、甘草;第2类为生地黄、牡丹皮、山药、山茱萸、泽泻;第3类为女贞子、墨旱莲、半夏、浙贝母、黄芪、三七、蒲黄;第4类为菟丝子、枸杞子、车前子、熟地黄、川芎;第5类为丹参、郁金、五味子、党参;(4)关联分析:关联强度高的药对组合有,菟丝子-熟地黄,菟丝子-女贞子,车前子-菟丝子,菟丝子-当归等。因子分析可提取出10个公因子。结论AMD的中医治疗多从肝、肾入手,重在滋补肝肾、化瘀止血、利水渗湿等。 OBJECTIVE To explore the rule of Traditional Chinese Medicine(TCM)in the treatment of age-related macular degeneration(AMD).METHODS The documents were searched with CNKI,Wanfang,VIP,SinoMed,PubMed,Embase,MEDLINE,and Web of Science databases as sources,and the date was set from January 2000 to September 2022.According to the inclusion criteria,the included documents and extracted data were entered into Excel to establish a database,and SPSS Statistics 25.0 and IBM SPSS Modeler 18.0 statistical software were used for analysis.Chinese medicines included in the prescription were statistically analyzed.RESULTS(1)Chinese medicines and efficacy classification with high frequency:A total of 147 articles were included,and 152 prescriptions were extracted.Among them,the top three Chinese medicines with the highest frequency were wolfberry fruit(65 times,42.76%),poria cocos(63 times,41.45%),and angelica sinensis(61 times,40.13%).According to the classification of efficacy,there were seven categories of Chinese medicines involved.The top three categories with the highest frequency were tonic drugs(505 times,50.91%),diuretic drugs(117 times,11.79%),and blood activating and stasis removing drugs(116 times,11.69%),respectively.(2)Chinese medicines property and meridian tropism with high frequency:Among the properties,cold(11 times,39.28%),warm(nine times,32.14%),flat(eight times,28.58%)accounted for a high proportion,with no thermal drugs;In the statistics of drug taste,the proportion of sweet(18 times,42.9%),bitter(ten times,23.7%)and pungent(eight times,19.0%)was high,and there was no salty drug.In the statistics of meridian tropism,the liver meridian(18 times,24.7%),the lung meridian and the kidney meridian(12 times,16.4%,respectively),and the spleen meridian(11 times,15.1%)accounted for a high proportion.(3)Cluster analysis:Chinese medicines with high frequency are divided into five categories,and the first category includes:Poria cocos,Atractylodes macrocephala,Dried tangerine peel,Angelica,paeony,Bupleurum chinense and licorice root;The second category includes:Raw rehmannia,Peony bark,Chinese yam,Cornus officinalis,Alisma orientalis;The third category includes:Ligustrum lucidum,Eclipta,Pinellia ternate,Fritillaria thunbergii,Astragalus membranaceus,Panax notoginseng,Typhae typhae;The forth category includes:Cuscuta chinensis,Lycium barbarum,Plantain seed,Rehmannia glutinosa,Ligusticum chuanxiong;And the fifth category includes:Salvia miltiorrhiza,Turmeric,Schisandra chinensis,Codonopsis pilosula.(4)Association analysis:There were drug pairs with higher correlation strength:Dodder-rehmannia,dodder-ligustrum lucidum,plantain-dodder,dodder-angelica,etc.Factor analysis could extract 10 common factors.CONCLUSIONS TCM treatment of AMD mostly starts from the liver and kidney,focuses on nourishing the liver and kidney,removing blood stasis and stopping bleeding,clearing damp and promoting diuresis,etc.
