摘要
目的探讨在腹腔镜阑尾切除术中采用双重套扎线处理阑尾根部的临床应用效果。方法选择2020年4月至2022年4月间在榆林市第一医院行腹腔镜阑尾切除术的247例患者的临床病历资料进行回顾性分析。根据阑尾根部处理方式的不同,将患者分为双重套扎线结扎组、单纯丝线结扎组和可吸收夹夹闭组。比较3组患者基线资料、术中及术后恢复情况。结果247例患者中,双重套扎线结扎组患者57例、单纯丝线结扎组43例和可吸收夹夹闭组147例。3组患者的总手术时间、出血量和住院总费用差异均具有统计学意义(P<0.05),其中双重套扎线结扎组的手术时间相较于单纯丝线结扎组显著缩短(P<0.05),双重套扎线结扎组的术中出血量相较于单纯丝线结扎组和可吸收夹夹闭组显著降低(P<0.05);双重套扎线结扎组的住院总费用相较于单纯丝线结扎组显著升高,但相较于可吸收夹夹闭组显著降低(P<0.05)。3组患者在术后并发症发生情况差异无统计学意义(P>0.05)。结论双重套扎线结扎、可吸收夹夹闭、单纯丝线结扎处理阑尾根部均安全可靠,采用双重套扎线有助于缩短手术时间,减少术中出血,且操作简便,利于临床推广。
Objective To investigate the clinical effect of laparoscopic appendectomy with double-strand ligation for the treatment of appendix root.Methods The clinical data of 247 patients who underwent laparoscopic appendectomy in Yulin First Hospital from April 2020 to April 2022 were retrospectively analyzed.According to the different treatment methods of appendix root,the patients were divided into three groups:double-strand ligation group,filament ligation group and absorbable clipping group.The baseline data,intraoperative and postoperative recovery of patients in the three groups were compared.Results Among 247 patients,57 patients were in the double-strand ligation group,43 patients in the filament ligation group and 147 patients in the absorbable clip group.There were statistically significant differences in total operation time,blood loss and total hospitalization cost among the three groups(P<0.05).The operation time of the double-strand ligation group was significantly shorter than that of the filament ligation group(P<0.05).The intraoperative blood loss of the double-strand ligation group was significantly lower than that of the filament ligation group and the absorbable clipping group(P<0.05).The total intraoperative hospitalization cost in the double-strand ligation group was significantly higher than that in the filament ligation group,but significantly lower than that in the absorbable clipping group(P<0.05).There was no significant difference in postoperative complications among the three groups(P>0.05).Conclusion The treatment of appendix root by double-strand ligation,filament ligation or absorbable clipping is safe and reliable.The use of double-strand ligation is helpful to shorten the operation time and reduce intraoperative bleeding,and the operation is simple and conducive to clinical promotion.
