摘要
目的:研究尿液白细胞(White Blood Cell,WBC)、红细胞(Red Blood Cell,RBC)检测中应用全自动尿沉渣分析仪检查与显微镜检查的效果。方法:研究对象为本院2021年9月~12月采集的500份晨尿样本,所有尿液样本均进行全自动尿沉渣分析仪与显微镜检测,观察比较两种检查方式的阳性检出情况及全自动尿沉渣分析仪检测尿液WBC、RBC的效能。结果:500份尿液样本中,显微镜检查WBC阳性检出率为14.60%,尿沉渣分析仪检查WBC阳性检出率为24.80%,真阳性率、假阴性率、假阳性率、真阴性率分别为12.20%、2.40%、12.60%、72.80%,以显微镜检查结果为金标准,则尿沉渣分析仪检测WBC的准确率、灵敏度、特异度分别为84.90%、83.45%、85.15%。500份尿液样本中,显微镜检查RBC阳性检出率为12.80%,尿沉渣分析仪检查RBC阳性检出率为19.80%,真阳性率、假阴性率、假阳性率、真阴性率分别为11.00%、1.80%、8.80%、78.40%,以显微镜检查结果为金标准,则尿沉渣分析仪检测RBC的准确率、灵敏度、特异度分别为89.40%、85.94%、89.91%。尿液样本中存在大量上皮细胞、非结晶尿酸盐、大量细菌、结晶、WBC形态破坏、影细胞、小红细胞等因素与尿沉渣分析仪检查检测假阳性、假阴性结果较多密切相关。结论:尿液WBC、RBC应用全自动尿沉渣分析仪容易出现假阳性、假阴性,但检测效率高、重复性好,在实际工作中可与显微镜检查优势互补,提高检测结果的可靠性。
Objective:To study the effect of automatic urine sediment analyzer and microscope in the detection of urinary white blood cells(WBC)and red blood cells(RBC).Methods:The subjects were 500 morning urine samples collected in our hospital from September to December 2021.All urine samples were detected by automatic urine sediment analyzer and microscope.The positive detection of the two inspection methods and the efficacy of automatic urine sediment analyzer in detecting WBC and RBC in urine were observed and compared.Results:Among 500 urine samples,the positive detection rate of WBC by microscopy was 14.60%,the positive detection rate of WBC by urine sediment analyzer was 24.80%,and the true positive rate,false negative rate,false positive rate and true negative rate were 12.20%,2.40%,12.60% and 72.80% respectively.With the results of microscopy as the gold standard,the accuracy,sensitivity and specificity of urine sediment analyzer in detecting WBC were 84.90%,83.45% and 85.15%,respectively.In 500 urine samples,the positive detection rate of RBC by microscopy was 12.80%,the positive detection rate of RBC by urine sediment analyzer was 19.80%,and the true positive rate,false negative rate,false positive rate,and true negative rate were 11.00%,1.80%,8.80%,and 78.40%,respectively.With the results of microscopy as the gold standard,the accuracy,sensitivity,and specificity of urine sediment analyzer in detecting RBC were 89.40%,85.94%,and 89.91%,respectively.The presence of a large number of epithelial cells,amorphous urate,a large number of bacteria,crystals,morphological destruction of white blood cells,shadow cells,small red blood cells and other factors in urine samples is closely related to the false positive and false negative results detected by urine sediment analyzer.Conclusion:Urine WBC and RBC are easy to appear false positive and false negative when using automatic urine sediment analyzer,but the detection efficiency is high and the repeatability is good.In practical work,it can complement the advantages of microscopy and improve the reliability of detection results.
