摘要
目的:分析类风湿性关节炎(RA)患者营养状况及其与免疫功能的相关性。方法:选取2021年2月-2022年3月齐齐哈尔市第一医院收治的RA患者70例为观察对象,按照类风湿性关节炎疾病活动性评分(DAS28)进行分组,以3.2分为分界线,>3.2分为活动组,≤3.2分为低疾病活动组,各35例。比较两组患者免疫指标、生化指标水平,分析免疫指标与生化指标的相关性。结果:活动组患者血红蛋白(HGB)、白蛋白(ALB)、前白蛋白(PA)、总胆固醇(TC)、高密度脂蛋白(HDL)、铁(Fe)、锌(Zn)、视黄醇结合蛋白(RBP)水平低于低疾病活动患者,差异有统计学意义(P<0.05);活动组球蛋白(GLO)、免疫球蛋白G(IgG)、免疫球蛋白A(IgA)、免疫球蛋白M(IgM)水平高于低疾病活动组,差异有统计学意义(P<0.05);IgM与HGB、ALB、PA、TC、HDL、Fe、Zn、RBP无相关性(P>0.05);HGB、ALB、PA、HDL、Fe、RBP与DAS28、GLO、IgG、IgA具有相关性,呈负相关(P<0.05);GLO、IgA与Zn具有相关性,呈负相关(P<0.05)。结论:RA患者的营养状况与健康人群具有较大差异,蛋白质指标、微量元素均紊乱,RA患者营养状况不良,可能与免疫功能紊乱、疾病活动度有关。
Objective:To analyze the nutritional status of patients with rheumatoid arthritis(RA)and its correlation with immune function.Methods:70 cases of RA patients admitted to Qiqihar First Hospital from February 2021 to March 2022 were selected as observation subjects.The patients were grouped according to the disease activity score of rheumatoid arthritis(DAS28),with a cut-off of 3.2,>3.2 as the active group and≤3.2 as the low disease activity group,35 cases each.The levels of immune indexes and biochemical indexes were compared between the two groups of patients.The correlation between immune indexes and biochemical indexes was analyzed.Results:The levels of hemoglobin(HGB),albumin(ALB),prealbumin(PA),total cholesterol(TC),high-density lipoprotein(HDL),iron(Fe),zinc(Zn),and retinol-binding protein(RBP)were lower in the active group than in the patients with low disease activity group,and the differences were statistically significant(P<0.05).The levels of globulin(GLO),immunoglobulin G(IgG),immunoglobulin A(IgA),and immunoglobulin M(IgM)were higher in the active group than in the low disease activity group,and the difference was statistically significant(P<0.05).There was no correlation between IgM and HGB,ALB,PA,TC,HDL,Fe,Zn,and RBP(P>0.05).HGB,ALB,PA,HDL,Fe,RBP were correlated with DAS28,GLO,IgG,IgA with negative correlation(P<0.05).GLO,IgA were correlated with Zn with negative correlation(P<0.05).Conclusion:The nutritional status of RA patients has a large difference from the healthy population,protein index and trace elements are disordered,and the poor nutritional status of RA patients may be related to immune dysfunction and disease activity.
