摘要
目的采用系统评价的方法分析针刺对血管性痴呆动物模型鼠脑组织内神经递质的影响。方法检索建库至2022年6月的中国期刊全文数据库(CNKI)、维普期刊数据库(VIP)、万方数据资源系统(Wanfang)、PubMed、Web of Science数据库中关于针刺干预血管性痴呆动物模型的实验研究,并且提取文献资料并进行质量评价,采用统计软件Review Manage 5.3对所收集的数据进行统计分析。结果纳入7篇文献,样本184例,主要结局指标经Meta分析结果显示:较模型组相比,针刺组血管性痴呆模型鼠脑组织内神经递质5-羟色胺(5-hydroxytryptamine,5-HT)的表达水平(SMD=1.43,95%CI[0.83,2.00],P<0.01)、去甲肾上腺素(noradrenaline,NE)表达水平(SMD=2.09,95%CI[0.58,3.61],P<0.01)、多巴胺(dopamine,DA)表达水平(SMD=1.75,95%CI[0.74,2.75],P<0.01)明显较高。因纳入研究中仅2项报告脑组织乙酰胆碱(acetyl choline,Ach)的表达水平,1项研究报告了r-氨基丁酸(r-aminobutyric acid,GABA)的表达水平,样本量过少,未作Meta分析。次要结局指标Meta分析显示:针刺干预组的血管性痴呆模型鼠水迷宫逃避潜伏期较模型组明显缩短(SMD=-3.07,95%CI[-4.94,-1.2],P<0.01),跳台试验错误次数较模型组明显减少(SMD=-3.72,95%CI[-6.72,-0.73],P<0.01)。结论针刺能够提高血管性痴呆动物模型鼠脑组织内神经递质5-HT、NE、DA的表达水平,但结果仍需要大量实验进一步验证。
Objective The effects of acupuncture on neurotransmitters in the brain of vascular dementia rats were analyzed by systematic evaluation.Methods Randomized controlled trials of acupuncture intervention on vascular dementia animal models were searched from CNKI,VIP,Wanfang,PubMed and Web of Science databases established until June 2022.The literature was extracted and the quality was evaluated,and the statistical software Review Manage 5.3 was used for statistical analysis.Results Seven literature were included with 184 samples.Meta-analysis of main outcome indicators showed that:compared with the model group,the expression levels of neurotransmitter 5-HT(SMD=1.43,95%CI[0.83,2.00],P<0.01)and NE(SMD=2.09,95%CI[0.58,3.61],P<0.01)and DA expression level(SMD=1.75,95%CI[0.74,2.75],P<0.01)were significantly higher.Since only two of the included studies reported the expression level of Ach in brain tissues,and one of the included studies reported the expression level of GABA,the sample size was too small and no Meta-analysis was conducted.Meta-analysis of secondary outcome indicators showed that:compared with the model group,the water maze escape latency of vascular dementia model rats in the acupuncture intervention group was significantly shortened(SMD=-3.07,95%CI[-4.94,-1.2],P<0.01),and the number of jumping test errors was significantly reduced(SMD=-3.72,95%CI[-6.72,-0.73],P<0.01).Conclusion Acupuncture can regulate the expression of neurotransmitters 5-HT,NE and DA in the brain tissues,improve the ability of learning and memory in vascular dementia model rats.
